In descriptions on this item is something like that Penalty: Using more than one type of this module or similar modules that affect the same attribute on the ship will be penalized.
But i see in few fitting 3 or 4 items like this on one ships
I have question what penalty is when i use three item like this on one ship

e. We fit the 4th damage mod on setups where DPS can't be compromised for silly things like tanking ;-)

Well numbers aren't correct but its something like that. If you go all out gank you still want 4.
If you want maximum buffer and dps, good balance. You should only have like 2 magnetic field stabs and only 2 Energized adaptive nano membranes which have the same kind of penalty but for resists.
To explain it very roughly and shortly.

You get diminishing returns on the bonus. That's the penalty.
So, if you have two mods bonusing rate of fire by 20% you don't get 40% you get less than that.
Here's the formula:
(mod1*mod2*...*modn)^((1/n)^0.25)
where 'n' is the number of mods that effect that attribute.

This is actually the old stacking formula. It was changed in 06 afaik. old thread

It still does *something* just not that much. The question then becomes "Is there something more useful I can use that slot for?".
Often there is but it's a judgement call rather than a hard and fast rule.

Most "sensible" gank fits use 3, sometimes only 2 damage mods (Or 2 rigs instead of one damage mod). 4 damage mods are for special occasions and lolEFT fits. (I sometimes do 3 Magstabs and one T2 Hybrid Burst aerator rig though, the stacking works thus that you get a bit more mileage out of that rig... But even that is not cost effective at all, 40+ mil for maybe 40-50 more DPS :P ) ---
