I'm going to throw it out there!
How do people make billions and billions of isk?
I have tried mining, mission-running, invention and various 0.0 variants, but I can't seem to get past much more than 1-2 billions.
Are the people that buy and use supercarriers and titans paid by an alliance? moon-mining? lvl5 mission solo?

I buy the ISK directly (PLEX) and fly somewhat cheap ships. What I don't lose in PVP, it's almost offset in L4 missioning.
Screw the grind, of any kind. -other than time itself for skills.

To make billions of ISK, find ways of making other people earn it for you. Do not mine yourself when you can pay others a pittance to mine that stuff for you. Do not haul stuff yourself when you can pay others a pittance to haul for you.
Expand your trade and manufacturing capabilities, make a teensy amount of profit from a huge volume of trade. End up with a fat wallet. -- [Aussie players: join ANZAC channel] |

Besides the more passive ways there are several pve centric ones as well. FW lvl4's are one, they net you an insane amount of cash if you do some research into it for relatively very little time involved. Also look into your options to reduce your isk/time spent buying and trying to buy stuff. Courier contract services to haul stuff around for you and once at destination (say market hub) you can remotely contract them from where you live etc. Also getting into contact with people who produce ships who will often sell for a good margin below market price and deliver it to where you want.

Two things:
1) The people who are making billions have figured out how to do it over a period of time - or had some one tell them. Usually there's a period of capital accumulation. It takes money to make money. They may have several things going at once - and they may have alts to support them - so that the effort may not be just that of one character.
2) For really big ships those are mostly the product of corporations and alliances as ... you need support to use those big ships ... so you may as well use that support to get them.
Different people play the game for different reasons and some of them like making a lot of money - so that's what they spend their time figuring out how to do. Other people don't care so much about how much money they make - as long as they have enough to do the things they enjoy doing.

The game doesn't require you to have "billions and billions of ISK" to enjoy it.

I made about 500 million ISK and ~260k LP running missions in hisec over the last 2 days.
Last summer I did anomalies for a couple of months (the server was too fail for any PvP) and made about 16 billion doing that.
Those are relatively effort-intensive ways of making ISK but even so I didn't have to focus more than a fraction of my attention on them. And that's the trick really, find something to do that you can tolerate doing a lot of, and then the ISK rolls in. Signed, Pheusia |

1)find idiots 2)pay idiots to do menial tasks for a pittance (but enough that they think you're awesome) 3)sell what they make you 4)Profit
Specifically off the top of my head
Setup a number of pos's to mine moons, Find some meat sacks to fuel them for you, Pay them enough to make them think it's a sweet deal little enough that you're making a profit. Sell processed moon goo for profit?
Same applies to Planetary interaction stuff, Pay people to mine the base materials for you then throw them all into a production planet with huge bins on it, come back in a day or so and sell your entire product.

It pays to look into "passive incomes". Things that earn you cash while you go and do something more interesting. Typical example: Planetary Interaction. They need some maintenance every day, but as a thank you, they work 24 hours a day. If you get all characters on you account fully trained, that's 18 colonies working for you. Something as simple as this will give you a passive income of a few hundred million / month.
Moon mining of course is another passive income, R&D agents yet another, trading in bulk can be yet another.
The best part of course, is that while passive jobs work, you can go do something else - like missions or mining.
Double income! (What does it mean?)

I do believe that for most billionaires they have taken the long road with the markets. Set up buy orders, then set up sell orders when you get the stuff. Log on at least once a day to check your orders, and reset them so they're on top of the pile. At busier trade hubs and with more commonly used items you will see your isk turned into goods and back again faster, but have smaller profit margins, and you'll have to check your orders more often.
This trader stuff results in a exponential growth, i.e. your income is related to the amount of wealth you put into it. If you reinvest your net profits your wealth accumulation will accelerate. But even then you can't expect to become a billionaire overnight.
Of course there is also manufacturing without mining. Look at the BPOs, do the math on them, and then research then extensively. EveMon has tools to help with this. Set up buy orders for the raw materials, haul them a few jumps away from the tradehub to a station with manufacturing lines, build, haul it back to the trade hub, sell order. Rinse repeat ad infinitum. Though you should train Production Efficiency Lv5 first, a couple weeks for that. Tech 2 stuff requires even more training, and it's more complex, but it does have higher profit margins.

No idea how people make and keep billions. The bolded part is the bit I think is important. I have no trouble making money, but I always seem to go on a spending spree when I get to about 600 million ISK.

I say - be careful what you wish for - you may find having tens of billions of ISK may spoil the game for you.
Two things
1. Run level 4s and max some social skills to increase you ISK and LP reward
2. Trade - read any of the guides on the market forums - put it into practise with small cheap items - discover you can trade and it is easy - invest in more expensive items - trade skills - diversify - go to bed - wake up next morning with an additional 100mill in your wallet
3. Do 1 and 2 at the same time
4. 1000 other ways to make ISK in this game - just find something you like doing and be good at it!

Set a level below which you consider yourself "poor". Make ISK until you have this much left and the ships you need.
When you dip down to your "poverty line" again, make ISK.
