They don't need to be of the same level, just for the same corporation.

totaly wrong they don't need to be same corporation, just same level of agents from same FACTION

i take it nobody knows for sure.

Yes, we do know for sure.
It's 16 missions for the same faction, of the same level.
Where you do your 16th mission determines where the storyline agent offer is made. It'll be the closest one from that system. In case of a distance tie, it seems to chose a random agent. I've done my 16th mission for an agent in the same station as the storyline agent I wanted to use, but I got the offer from a storyline agent in another station in the same system, so it seems like it only cares about system distance.

assuming you don't already have a storyline mission open with that agent - If you do have a mission pending they will use a different agent.

As I hear it, the count resets for that faction and level if a rejection/failure of a normal mission results in a standing loss. That is, you can reject an unaccepted mission for a given agent every four hours without consequence. If you do it more often, or if you fail an accepted mission, I think the count resets to zero.

From my experience: you need to make 2 more mission to cover 1 failed mission.

Never been there (some were listed as level 1 agents but required high standing [the agents offering the ship BPC]).
The low level Gallente Cosmos agents are: Caldari and Truckker.
Substantially, Gallente Cosmos weren't completed when deployed and the Cosmos project in high sec has been stopped a lot of time ago.
