I've just finished reading the Nights Dawn trilogy by Peter F. Hamilton. I know, I'm behind the times, but it is a truly astounding work and highly recommended to anyone playing Eve.
Anyway, one of the key mechanics in this particular SF Universe is that in the vacuum of space ships have massive amounts of trouble dissipating heat. Which actually makes sense. Although space is very very cold, it also a near total vacuum. And as anyone who has ever owned a Thermos flask will know, a near perfect vacuum is a near perfect insulator.
In the books, getting rid of heat required the deployment of ungainly heat sinks on the shadow side of the ship. Heat sinks which couldn't be used in combat, or when moving at high velocities, or when attmepting to be stealthy. When not deployed, all the mechanical and thermal functions of the ship, plus any additional radiation from nearby stellar bodies, weapons fire and detonations, and particularly stars, would have a constant heating effect on the ship. Allow the ship to heat up for too long and you start running into problems.
So what if a similar effect happened in Eve?
Activate a cloak and your ship starts to heat up. Run around with an MWD or AB on, your ship starts to heat up. You get the picture.
I really dont know how you'd go about integrating this. Possibly with the current heat system. Sit around cloaked all day and all your racks will be at maximum heat. Soon as you switch on some modules, boom, overheated and shut down.
Proximity to stars would also have an adverse effect on your heat management.
Anyways, not really a fully formed idea, but i think that adding external factors to ship performance could be interesting step. Atm, there are no "Environmental" factors in Eve. You could have "Hot" systems, binary stars for example making heat management a significant factor in a battle. Or Pulsar or Quasar systems with unusual background radiation that can have a negative impact on ship system performance. With such environmental effects causing problems for all protagonsists equally, player skill in reducing their effects could become another factor in battle. Skills that would need to be developed independant of skill points.
How cool would it be. 2 fleets clash against the background of a glaring binary star system, ionizing radiation causing hell with modules, possibly causing resist reductions or massive heat spikes, even before a shot has been fired.
And if nothing else, it would give the CCP artists an excuse to prettify some star systems

The OP is mistaken about heat transfer. It's true that little will be transferred by conduction or convection, but radiation of heat is perfectly possible in a vacuum. Or has the way stars work changed since I last looked?
