One way to add some unpredictability in the game would be to introduce a chance that modules would break on your ship while using them. They'd go offline for some random duration while repairs are preformed, after which the module would come back online and be ready to be used. There would be of course a skill in the Mechanic section that would either reduce the chance of modules breaking in the first place (call it 'Maintenance'), and another skill that would reduce the time the module is offline while broken (call it 'Repair').
As a carebear myself, it would add some challenge to the boredom of missions if, say, my booster were to go out in the middle of the AE bonus room.

I've seen similar suggestions on this subject and have thought about ways to make this more appealing. Tuning of modules (like a tiny but constant overheat), improving the stats of the module by a small margin and/or lowering the fitting requirements. A highly "tweaked" module would have a larger chance/risk of breaking down while a stock T1 module would behave as it does today.

however, i suspect most people woudl find that annoying, and then petition it when their ship died in a hole.

Well, if CCP were to listen to all the people finding stuff annoying, we'd end up playing EVE: Hello Kitty Space Adventure or something. Like with everything else, there might be some grumbling at first, but in time it becomes part of the game and people get used to it. I just think it would be a cool game mechanic, and along the lines of the concept of targeting specific ship modules in combat (though a heck of a lot simpler to implement I imagine).
Please keep the comments coming.

well i suggested a while ago that all emp wepons and ammo or what ever get renamed and ad emp smart bomb or bomb or something simular.
this device would be a last dich effort modual and maybe fitted on a new class of t2 destroyer.
this device when set off is friend or foe much like a dooms day device. if a pilot initiates such a device all ships affected has all ship moduals deactivated.
i would say sensor boosting moduals should grant a chance to avoid a full shut down. the more eccm fitted to ship will improve your odds of having each modual remain active.
is there people haveing problems with ecm? well this idea would give you another reason to fit eccm to avoid them pesky ecm bombs or jammer ships hehe

I'm not sure what an EMP bomb has to do with my suggestion. What I meant is to have modules break temporarily because stuff IRL breaks down during combat, and has to be either replaced or repaired. My suggestion would simulate that effect, and would add some small measure of unpredictability to the game. CCP is going through some lengths to add such unpredictability using the Sleepers to keep the game vibrant, so I think it's a goal for them. This would simply be another method of doing so.

To be honest one of the reasons i play eve and enjoy it is because when i have other things to do around the house or whatever i can still do something productive on the game, if i want to watch the screen at all times then i'll go to 0.0 or low sec :)

Basically you're suggesting to add random annoyances to the game. You could also achieve this by randomly disconnecting players. Would also conserve bandwidth. Pewpew |

I do as well, but that's turned EVE into less of a game and more of a grind. CCP has already hinted at introducing more randomness into the game. They're adding Sleepers, and eventually they said Sleepers will randomly pop into your space even outside W-holes to add that unpredictability. They're also re-doing the NPC AI for the same reason. This is simply an additional suggestion along those same lines, and I think would take less coding to implement.
I do think the days of AFK EVE are coming to an end, or at least being reduced, so we might as well all get used to it.

This. EVE Online is a rare breed in the sense that there are things you can do in the game that you can do while doing some out of game things at the same time. The amount of things is limited (as it should be) and they're not the things that will yield the biggest rewards, but they are there, and I really like it that way, too.
The idea the OP is suggesting is something I too would find rather annoying. Being attacked by a spawn of NPC's is one thing, but having to check whether all the modules of your ship are still working sounds like a rather boring and annoying chore. I think we should be able to depend on the ships and equipment we use, unless we ourselves choose to engage in combat and/or overheat them for example.

Point taken. I agree that this suggestion would be an annoyance to people who want to play EVE without actually playing EVE. For many others who actually enjoy being involved in the game, I think this feature would add a nice measure of realism.
