We like to employ stupid people, here's my story.
We mine in 0.6:
me in an apoc, one hauler and a thorax. I have 2 shooting lasers equipped, the rest miners. Everything goes well, i was talking to my potential new girl friend on msn, which got me distracted.
Someone delivers my groceries and because i live on the third floor i need to run down to get the groceries. I run down, put the groceries in the kitchen and get back to talking to my new playboy bunny on msn. All of a sudden i get a phonecall, its my rl friend kuntuna, he says...
- I think you're about to loose an apoc.
I go : how ? I'm not even playing ?
- Uhm you were mining remember ?
I got WTF NOOOOOOOOOOOOOOOOO........
Just in time... To see 6k rats blow up my uninsured apoc (pre-40% payout when not insured)

This little one i done on christmas day.
Still half cut from the night before i logged on to check if / when my skill finished training. Had the whole family thing going on so was in a rush aswell.
I then decided the exit station button was infact the exit game button and at that point my hauler containing loads of stuff i was going to transport to a new base (at another time) was thrust out of the station right in front of a group of about 10 CA pilots who just happened to be attacking the station - doh! 
Lag combined with sheer disbelief at the stupidity of my actions resulted in both ship and pod going bye, bye. 
Needless to say they had a Merry Christmas since the hauler had loads of named and tech 2 mods, plus two advanced learning skills and a shed load of cruise missiles and other stuff

OMG. where to start ?
- Lost covert ops ship to gate sentries, cos i locked and fired at a noob ship scout in 0.0 empire space.
- Saw baddies at gate and thought. christ. best warn people. there may be a gank squad about. and jumped thru to aforementioned gank squad.
- Lost more ships in total to station/gate sentries than enemy ships due to locking the wrong gate/station or friendly ships.
EDIT...oh yeah, and lost megathron cos i shot someone's abandoned drones cos i was bored on a mission, then warped to gate, and sentries started firing. Odd I thought. better get away. so jumped thru gate...... concord were waiting!!!
I could go on. I am a cluster waiting to happen. Lol. If you see me... stick around, you'll laff yer head off

Told my very n00bish corpmate to lock on someone stealing our ore while mining. He did, waited until the indy had turned around and then opened fire.
I ordered a swift lobotomy and figured, why should he have all the fun and opened fire as well. That was stupid. We both died naturally. I go back to the station in my pod and decide I should atleast pick his things up since I felt responsible. Get into my Maller and undock. Hello sentry guns.
That made me feel ever so slightly retarded ...

I needed some isk to insure a BS so I thought I would sell off a arblest launcher.
I make the escrow, and it sells almost instantly...
Wow I think, I had put it up at what I thought was a rather high price. Then I remember that the number of an item doesn't show in the escrow screen any more.

A corp mate and I were killing this Freelancer guy somewhere in cache a while back. When the megathron blew up I opened the can, which had 6x 425 Prototype rails in it, with a bunch of other really good named stuff in it. Before I could grab anything tho, the can exploded! I was like WTF!!! I ask what happened- "I had to get the pod" my corpmate goes... He used his smartbomb to take out the pod, which killed a can worth several hundred million isk...
"At least I got the pod" he goes, haha

I had been in the game for abuot 3 monthes and went into PF-345 in a thorax. ended up in a pvp agaist squirrel he was in a scorp... i lost. scorp v thorax... should of known better learnt from that mistake

1/2 year ago I was hunting rats in placid region, a pirate jumps in on me and started to shoot at my scorp. I started to warp away, then noticed it was only kestrel. I locked him down and scrambled him, then my ship warp out...
and on xmas I bought 1 graviton reactor for 55 millions... someone got a nice gift. First time Ive got scammed. lucky for me I only needed 1

my first real pvp experiance launched 1 volley off torps in my moa (way long ago) allmost wtf owned the hauler but forgot too turn on auto repeat hehe
lost a 1 day old megathron too rats in some unknown system ratting . i just knew i should have logged lol . but thought just another 2 secs . b00m =p
lost a typhoon . thinking wonder if tank ceo will shoot me . instead of omfg run lol.
