Alright, the year is at an end. Post the three biggest fails in eve you've seen this year. Here are mine:
1) Fail trap: A megathron challenged me to a BS 1v1, I accepted. Once the fight started he brought in 2 more BS's and a pilgrim. But I have friend too, they lost all but the pilgrim. We lost nothing but ammo and cap boosters.
2) Carebear fail: There he was in a carrier, running a lvl5 in my home system without paying his taxes. We probed him out with one of the worst probers I've seen (no, it wasn't me) and he didn't see us coming. We landed 40km from him and easily tackled him with BS's despite all of us being MWD fit and no MWD's allowed in the mission. And what's worse, he killed everything in the mission except the mission objective (a structure)! He could have run that mission 2-3 times before we even had him probed down if he had only bothered to read the briefing. Instead he failed the mission and lost his ship.
3) Fail bait: A vengence attacked my ashimmu on a station (I'm an outlaw). I knew he had 3x guardians, 2x vigilents and a zealot next door waiting. I also knew it was a 20+ au warp from the gate to the station and he was active tanked. 90% web + 300 turret dps + 3x medium neuts = one very dead vengence long before his buddies dropped out of warp.

1) A certain 0.0 entity I wont name here poked their heads into our little area of lowsec through a wormhole. We obviously investigate, and it turns out they are camping their side of the wormhole. A little skirmish ensues, and despite having two carriers on field they manage to lose about 2 billion in ships, among them a CS and a BO, before they drive us back by dropping 3 more carriers and a titan. They killed a falcon and an arbitrator though.
2) A Vagabond pilot lands in our little camp on a hisec gate. Not even bothering to shoot him first, for some unknown reason he thinks this is a great opportunity to smack a bit in local before jumping. He gets bumped while typing, and dies some 200m short of jumping. Was not the first Vagabond he lost to our camping efforts in a hilarious fashion either.
3) Some guys try to bait two of our BCs with a Pilgrim in a belt. An active repped, 800mm plate pilgrim, no trimarks. It didnt work out so great, he gets pointed from 50km away by our Arazu, and slowly worked down from range without being able to point anything. At some point, his mates decide to explode local to save him, arriving too late to even see us warping out after looting the wreck.

Moros with a large armor repper, t1 large artillery and t1 drones + cloak. Bumped off station and killed by three BS and a Curse.
He died because he engaged a Ferox on that station. á á

1) Fail pirate: drops a Tengu strategic cruiser on my BC for "fun". Doesn't bother to even loot the ship after the kill. Pathetic. Get a hobby.
2) Fail pirates: pops an empty hulk in a gate camp to enlarge their epeens, but lets my industrial loaded with 60 million isk worth of robotics and megacyte through without even targeting it. Ganker ****s. Useless.
3) Fail event: Hulkageddon. An embarrassment for all involved.

1. Fail piracy: Two guyes from RA in her noble goal to gank my pest during doing anomalies with a arbitrator and cane. I think the moment they did realize that they became the pray instead of the hunter, the arbitrator pilot was already in a pod and the cane was pointed and caped out. I really love the pest for PVE.
2. Fail capital use: For warping a Tanatos to 100 at the sun, fit it with armor and shield tank and doing this in a system where quite some pirates are around.
3. Fail antipirating: A untanked Carcal, Omen and Vaga tried to gank my pest under sentry agro, couldn't even lock the Carcal before it died to sentries and was a bit unlucky that the vaga slipped a bit out of point and neut range while I tried to ***** myself on the Omen mail before sentries toasted it. I was actually sure a blob was on the other side but there was nothing after I waited the 60s and checked it out.

I'm in FW for the Amarr Militia. I start in Amarr and set my AP for Rens (needed to be there for business), shortest route, which goes right through Huola-Kourmonen-Auga-Amamake, the hotbed of Amarr-Minmatar FW.
I'll figure no problem, autopiloted through those systems many times before, never suffered a lethal attack yet. Besides, I have sound on, so if I'm AFK and I hear warning bells, I can always rush back to my monitor, hit the afterburner and jump through the gate, right?
So, at some point I hear the beep of my shields going down, I think 'WTF?', run to the monitor, switch to EVE and hit afterburner. However, my ship hardly accelerates, crawling towards the gate on some 230 m/s. I look at my overview and indeed see a couple of hostiles there, but none of them red-boxing. WTF?
As my last armor peels off I suddenly I realize I'm already two jumps into hostile high-sec, and its the NPCs are killing me, not any players, and its the NPCs with their pre-nano-nerf high strength webifiers slowing me down and of course not appearing on my standard PvP overview. ____

Fail drake gank: (**** BHs) http://vip3r.eve-kill.net/?a=kill_detail&kll_id=7802526 I don't want to know how many drakes I've nuked, but this went differently. I started out in an arazu and as it was a blackhole + they were drakes I figured I'd go reship as soon as others had tackle because we needed some dps+webs to get things done before the sleepers shot us too much. Well the system effect meant the dualboxed bomber had to close range to hit and I couldn't get it out in time as agility was boned, and I then spent a good while wondering why my abaddon's damage was doing nothing either as the optimal range reduction isn't a noted effect in the reference I was using at the time. Eventually the drakes were apart, our ceo's tackle proteus was losing armour and we made a ...tactical withdrawl. New-to-the-corp ceo's friend (in a chased-off tackling covops IIRC) was not encouraged by what he'd seen.
Fail gank-the-gankers: (Props to them, they didn't just POS up and had they got the RR+cap chain sorted quicker it could have been very different) http://vip3r.eve-kill.net/?a=kill_related&kll_id=7625250 Note that for the most part it's 2 then 1 BS(dualboxed) and a BC vs 7 of them (>2bil isk's worth) with logistics, RR, etc. I was in the cov ops this time having been annoyed that my new cloaky proteus wasn't as good for scanning ships&sigs, but now found myself wishing I was back with all that armour & dps. The bears had finished a site, I'd snuck a few km off of a salvaging loki and proteus pair and call the guys in. They just missed also pointing the prot, I bailed once the loki was tackled and hurried to go & reship as the anom-runners came to defend their loot. Over vent I hear more and more threatening primaries being called, the logistics, the domi(is it packing neuts?!), the paladin (is its web stopping someone get out of point range?) but by the time I get back our 3 points have managed 7 kills for 1 loss. This time round I think the (different) new guy in the BC enjoyed it. I'm still miffed I missed the orgy of destruction but glad I could provide it.
Fail capship w-space plans: http://vip3r.eve-kill.net/?a=kill_detail&kll_id=8346500 It started out with some corp hauling via a c3 with a static highsec which turns into highhole timer + mass games (gay, almost immune ships, need of bubbles & webs & bumps which we didn't have this time). Their orca in highsec can just jump back out if it comes in, our cloaked orca needs to wait out a session timer and warp away from the w-space hole if it wants to go out and in again. We try to make something of the situation and show our orca, sending it out, and they jump theirs in to trap it out but the hole's gone crit. Our fast-thinking cloaked mach pilot finishes the job killing the hole before session timers are up and a wild t3 appears to start chewing on their orca. The thanny at their pos starts moving just as we'd hoped, and the greater trap is sprung. While our mach pilot goes 14 jumps across highsec in a(n agile) BS to come back in to this c3 via another hole, our 2-3 dps ships get to work on the 2nd cap ship. An... interesting scorpion death later and a nag is sighted and lands 100km off to try sniping our neut BS. It just gets away again in time as a bonus-point-ranged proteus chases it down. Sadly for your killmail viewing pleasure, we were seemingly a little too intimidating for it to warp closer and demonstrate the infamous 'RR-dread plus now-in-triage-carrier' duo of death... I refer you to this local quote:

Mmm, I should add one more. I actually meant to put this in the initial list but forgot.
4) Probed out a missioning apoc, warped to it at 0 and was ****ed that he wasn't there anymore. Then I noticed his ship wreck next to me. The npc's killed him before I landed.
