I've only run ~20 high sec lvl 4 missions, but am yet to find any faction loot.
ie things like: DREAD GURISTAS CRUISE MISSILE LAUNCHER PITH X-TYPE LARGE SHIELD BOOSTER TRUE SANSHA STASIS WEBIFIER
Any help appreciated - thanks.

Deadspace loot you won't find in missions, except possibly from the overseer in Worlds Collide. For these you will need to run combat signatures that you scan down using core probes.

I'm not sure where these appear, I think they only drop from elite faction rats in belts as well as in the bonus room of Angel/Guristas Extravaganza.

Neither of those items will appear in Angel Extravaganza - even in the bonus room - because they are not Angel faction items.
Best chance of finding those items are in 0.0 ratting...

Dread Guristas Cruise Launcher will drop from a Gurista Commander spawn in 0.0 (Vale, Venal) belts. ie Dread Guristas Conquistador
True Sansha Stasis Web will drop from a Sansha Commander spawn in 0.0 (Catch, Stain) belts. ie True Sansha's Dread Lord. Although in my experience, webs and scrams usually drop from cruiser/BC spawns.

Not commanders. Commanders are Estamel and Brynn, and others. What you're on about are "faction" spawns. Or, to the locals, a "dread spawn" or "domi spawn", for guri and angel respectively. _____ ^ That is a sig line. It should be there without me having to put one in. |

I've gotten a Domination SML (gathering dust) and a small Gistii AB from escalations of high sec scanned plexes. The AB was from a low sec site in a system with only me and one other person in it. Got 120 Million ISK for that one.
There are rumors that faction loot drops from the bunker in the xxxxxxxxx Extravaganza bonus room, but I've never seen one.

These can happen in ANY asteroid belt, station, gate, or celestial, but are more likely in null sec belts. They can also occasionally occur in some missions and cosmic anomalies.
Cosmic signatures are the most reliable source however: Most cosmic signatures have a "faction" spawn in the final room (sometimes more than one). You can also find A, B, C and X-type items typically drop from the end "Boss" of cosmic signatures (the rarer are actually found in escalation complexes from completing probe-able cosmic signatures and/or anomalies).
Your "Commander" modules (Brynn's or Tuvan's or Brokara's, or .... modified modules) are found in officer spawns. These occur very rarely in null-sec belts and more commonly (but still rarely) in the very end "Boss's" of your toughest signature escalations.

Belt ratting in 0.0 is your best bet, and don't chain them.

Wrong. Faction spawns such as this are what's referred to as a commander. Estamel, Brynn, Raysere etc. are officers.

So, to find faction loot in high-sec, other than the really low probability WC & Blockade, I need to scan down complex's? I assume the higher the DED rating, the larger the enemy ships, then therefore the larger the faction loot item? (small/med/large/x-large)?

Faction (e.g. Dread Guristas) loot also drops in the places where you find deadspace (e.g. Pith) loot. E.g. the end structure in a 6/10 Guristas DED complex can drop Pithum A-Type modules, but can also give you also faction modules or blueprint copies (Gila pirate cruiser). The overseer cruisers in that complex also have a small chance of dropping faction items.
Non-rated complexes such a Guristas Military Base have a decent chance to spawn a faction (commander) ship as well, and when the site escalates, the new locations often have such a ship as well. And the overseer of the end location (if you get there) can drop both faction and deadspace loot.
But that's mostly restricted 0.0 content.

The only stuff thats restricted to 0.0 is the specific sites.
Faction (commander) spawns, escalations and faction/deadspace loot stuff applies to high and low as well.
You can even find commander spawns in belts in high/low sec. Additionally albeit off topic, hauler spawns can occur in low sec belts.

Faction items used to drop in missions ages ago, before CCP nerfed it. There is still a chance in rarer missions that escaped the nerf bat due to oversight, such as Covering Your Tracks storyline.
