Your best bet is to just create a petition and ask, the submission process has been very bad for years now. They have said they are taking steps to make the process smoother but they haven't done anything to provide the makers of alliance logos with clear and correct information. Even though it was sort of promised... *hint hint!*
The logo I ended up sending, which I think is the one that got accepted: Was a single layer file, with an alpha channel added. (To tell the game where the logo is transparent.)
If you want PM me your email in-game and I can send you my logo-file and you can use that as a reference of how they are supposed to look regarding file/layer structure. Also, if you do create a petition reference this thread, and the thread I made MONTHS ago to articulate your point and draw attention to the on-going problem.

PS: Resolution of an image file is directly related to its dimensions in pixels, it's only if you are creating a file using "real" measurement units like inches/cm's/mm's to tell a printer how big its supposed to be when you print it out, DPI controls the quality of that print and will increase or decrease the resolution of the file.
A 128x128 sized file doesn't have a DPI until you decide how big you want it in print. -

At this time there is no maximum limit on the file size. Within the second stage approval process they can edit the resolution of the image so that it properly appears in game. Also how big can you make a 128X128 file (Just waiting for Barney Stinson to jump in and say Challenge Accepted)?
On a serious note don't try to make it the largest file possible as it could be rejected as something that requires too much work.
Also I don't know what your timing was with checking the wiki, but the template and some of the text was updated yesterday around 15:00 UTC.
