I'm not so sure that it was just one person. Some people across EVE looked at a stargate, then the BM system, and thought "I wonder..."
Logic, really. You make a BM 15 km behind a stargate, then warp to it. All goes well, you land on the stargate. I had thought of the same thing before I even knew what an "insta" was.
I'm willing to bet it was TomB, though :P

And therefore to remove the problems of bookmarks have everyone warp to 0m at stargates. Problem solved. I win.

to keep things on topic for once (and not get filled with more talk about pro-anti insta talk):
Maybe CCP can dig deep into their logs and find out who made the first insta :D then people will know who to blame/praise for the idea XD

We have had gate instas since beta. We didn't need station instas in beta though because there used to be some cheat back than where you hold ctrl-alt-shift while you click dock to insta-dock!

The first "insta's", if I remember the story right, were actually used to collect cans when mining, not for travel.

not sure, but didnt we had variable warp in ranges?
I remember getting out from 2-50km from stations and objects.
anyway, you just fitted an mwd and you flown 5km/s

I mean i can remember using instas in beta, so hard to say who invented them, but i definetelly know who invented the insta-region sale...
Good ol' Madri. but afaik he stopped selling a long time ago after people just started copying his bookmarks...
However it was his invention and i still value the entire khanid set he sold me a few years back :-)
