PS2's "Shadow of the colossus". The objective? kill the colossus that prowl arround the land (yes several). It's somewhat a descendant of ICO in terms of base, but much more darker and dramatic (thus the increased usage of greys).
It's one of those great games that are praised by the critics but never hit the spotlight due to a complex story and an audience that is more addicted to "SHOOT HERE KILL THERE SMASH ALL!" and not something that requires more brain power or a sense of stetical beauty

System Shock 2, never got beyond the first section as it was too hard/scary.
Doom 3 tried... but it just got too samey, the logs of crewmemebers were good, but it always revolved around monsters jumping out of cupboards.

i loaded up Doom 3 at a friend's house for the first time (He'd just bought it), and i sat and played it for a while with 2 friends as advisors... It was dark, and we were in the house alone. Quite a few times i screamed my ******* head off like a girl.
Like once, had the machine gun out, but it was totally dark. Then hear moaning and shuffling. hmm. switch to flashlight. ZOMBIE RIGHT IN MY FACE AAAH SHOOT OMG KILL IT AAAAH!

The aliens from the first AvP game. They looked scary, made scary noises, were bloody fast and due to the way the game was coded could surprise you every time. There were some fixed enemy spawns, but for the most part, enemy generation was random, each time through the game the you'd get different numbers of enemies in different random areas. You never knew where they were going to come from or how many of them there would be. In AvP2, like most FPS games, once you've played it through a couple of times you can just auto-pilot through the levels knowing where every enemy is, you can often even fire the shot that will kill them before you even see them in some cases (e.g. fire a grenade at an area before the crossing trigger area that makes the alien pop out in that area, *bang*, dead before it knew what hit it etc.).

7th Guest scared the crap outta me when it first came out (i was pretty young) i could never play it in the dark cause i would be to damn scared.
Doom 3 got me pretty shakey too -------- Kill em all and let god sort em out

Also about the dogs from the first resident evil...uhh hell yeah . i remember when that happend i paused threw down my control and lcoked myselfe in the bathroom for like 10 minuets lmao. also yeah william berkin used to scare the crap outta me not to mention when doing mission 2 when tyrant 103 rammed through the walls.

One thing that made Doom III scary way more than the stupid "cupboard monsters" (which are what put me off the game too soon) was the sounds. With a decent subwoofer (and no, that doesn't include the crappy computer speaker sets) the environment felt rather... vivid, with all the hydraulic pumps and such making you twitchy as you were sneaking around waiting to be maimed at any moment The game just ain't the same with a proper subwoofer

blind dogs from stalker at first they just falow u keep distance atacks just to test how strong u are they cab falow long time tru regions in game waits till best moment and atacks
tbh most smatr monsters in game ever they tho can fell where char is and use team work to kill
tbh i find **** loads of good artefacts so i need to leave all my extra ammo behid so making way back to my lada blind dogs atacked me killed 2or 3 but was out of ammo so they nice and slow killed me
tbh that was scary

Well, I probably won't mention anything not said in this thread, but here I go anyways.
1) Resident Evil 2, arguably the best in the series and don't give me the RE4 crap . Man, I love that game.
2) AVP 2. Marines of course, especially the first two, three levels. The background sound of the Motion Tracker, the gas streams and flickering lights. Oh yeah, pure terror.
3) Doom 3, before you have any equipment except the flashlight. I always wondered why an Elite Marine, infiltrating an outpost that has been severely infiltrated by demons and demonic powers starts off with a flashlight, but hey, it works 
4) Although I've only played the Demo, FEAR springs to mind, rarely have I experienced something as horrifying when Alma suddenly materializes herself at the worst time, in the worst thinkable place.
5) The first Silent Hill. I won't even go into details here. Much respect.
Guess that's about all I can remember right now.

That game was just pure sex, scared me and my dad to hell.

If you are going to check out FEAR, make sure you can play it with good settings. If you have to put everything on low to play it, then you are going to miss out on a great game, and it won't be as scary as playing it with high settings.
Also, here is a quick guide to FEAR graphic settings, if you have a crappy comp, you should be able to get the most out of it.
CONTAINS ONE SPOILER (A picture of alma), But, if you have played the demo, it isn't anything knew, and it's already been talked about in this thread.

The giant spiders in Tomb Raider 2. Always creeped me out.....
If you want to go into non-monster bits, the scariest thing ever was one section of Silent Hill 3 which was actually scary enough that I had to turn the game off. Only time that has ever happened to me. ----------

Resident Evil: Nemesis.
I've never been so scared in my life as that, I must've skipped a beat or something.
You've just ran away from an unkillable mutant 8 foot tall zombie guy who's just finished eating your mates, you run past him into the police station and lock the door, several minutes later after wandering through the building, you hear a window smash.
You KNOW he's coming for you, and there's nothing you can do, he's invincible and will stick a tentacle through your brain if you give him half a chance.
So slowly heart beating you get to the top of the stairs, the screen goes black and the classic Resi scene were all you see is the stairs on a pitch black background, and it's silent except for you footsteps. You get to the bottom of the stairs expecting to see nemesis running at you down the corridor, or bursting out of a door further down the corridor. The one thing you do not expect, is for that window your stood right next to to crash inwards and a massive 8 foot tall thing to jump at you roaring and ready to eat you.
I swear, I'm never playing a game like that in the dark again. ------

Wasn't a wonderful game by any means, but I have yet to find a game that can quite match its style of creepiness. Scissorman and those 'ka-shink ka-shink' sounds he made with his 6-foot long scissors while chasing you after he found you staring at your newly departed friend whose entrails are on the floor...fun times. I need to ebay me a copy of that game, I miss it...
Also, the section in Resident Evil 2 where the licker jumps through the interrogation room window. Scared the ever-loving **** out of me the first time I saw it. Was half-expecting something to happen, but not that suddenly.
