I am wondering, what player generated ideas have actually made it into game?

Warp to zero.
For a long time CCP said that would never happen. And then everyone cried. And then cried some more. And now we have it.

Once upon a time, Khanid ships used a mix of lasers and missiles, and they sucked badly. Now, they are a good addition to the Amarr fleet.
I probably wasn't the very first person to suggest, but I was.. adamant(sp?) about the warp to zero, but autopilot as 15km. This made it into the game.
Also, the current UI is pretty much all player requested.
I'm not sure how long it was in the development phase, nor how many times it was requested, however, once I heard about the market changes, I did request folders to be added in the market quickbar.
As long as you follow a similar ADDIE model as far as requests and changes, and if it seems that it will be good for the game, and not game breaking, changes will be made.

Yeah, a handful of well-presented player ideas have been implemented, or at least something similar. Maybe they coincided with ideas CCP already had in the works, maybe not. Khanid Mk. II is almost certainly a good example of an excellent suggestion that influenced CCP. The drone control improvements were debated endlessly, and I'm sure CCP employees read and were influenced by some of those threads, to a greater or lesser extent. WTZ is another one where a lot of very good noodling happened on these boards, and the final result resembled some of the suggestions.
A lot of awesome ideas don't get onto the drawing board, too. Good ideas. Ideas like the "align to" button, the ability to load ammo in multiple weapons simultaneously, etc.
