How about the the cannibal holocaust "chick on a stick".. or as i like to call it, the human pogo. Insert large pointy pole into rectum and out mouth... leave to die and of course you are alive on insertion. google cannibal holocaust and look at images for an example.

"The death of a thousand cuts".
Basically you're bound and then an executioner starts cutting little slices off your body until you die.
Bit of leg ... slice of arm ... ooh, lets go for a chunk of your calf ...
This was real a form of death penalty in China way back, and families would often pay the executioner to cut at something important like an artery.
An expert executioner could make you die as fast or slow as they liked.

Basically the most gruesome death I can think of is in one of the scenes in "seven" where... they find this guy/girl tied to the bed in the hotel room, the guy/girl is firmly tied down so can't moove and is sedated so can't escape, fed intestinally but not enough to survive forever, and just let to vegetate and foul in his excrements and dying body ....for a loong long time ... He is also kept form diying rapidly by attaching intravenous antibiotics into his blood to keep the infections from procrasting "slowly" ...brrrr
that scene in "seven" always gives me the creeps, everytime I watch that moovie...
but to quote some others outta this thread "this is sick!"

That one was sloth. The man in the bed was a criminal with a record the size of a phone book. The moral of that crime was that the criminal was wasting his life in pettiness and immorality.
As long as we're on the movie though they've got several good examples.
Gluttony - being forced to eat yourself to death.
Pride - only works on vain people like Ryan S'couse. Cut off their nose and give them the option of suicide or savior but be scarred for life.
Greed - being forced to cut off a pound of your own flesh. How painful would that be.
Lust - yeah being ****** to death by someone with a 6 inch bowie knife attached to their appendage would not be the most glorified way to go.
Envy and wrath were the last 2 but it was on a personal note. The killer killed his wife out of envy and jealousy and in return the cop killed him in a fit of wrath.

Has to be quite painful to be skinned alive. I could also imagine a form of torture where you drown the victim and then resusticate him. Do it all over again. Star Trek Voyager had a cool episode with the "good" holodoctor threatening a crew member that he would render him incapable of losing consciousness so he would have to endure pain never before experienced by living being Don't know if it's possible but sounds inconvenient Using open flame on primary sexual characteristics of one's body could also be quite dreadful. "Too bad" you cannot repeat it after a while. Yeah, drwoning could be repeated basically on infinite occasions.

I once visited the Museum of Medieval Torture Instruments in Prague (good value for money, would recommend it). It didn't confine itself to torture; in particular, there were several means of execution on display as well, including the 'Cannibal Holocaust' spike mentioned above.
Of these, the most spectacularly gruesome was probably one that was displayed on the third floor, with a little woodcut depicting its traditional application. Here's a rough guide to how it worked:- Build a sturdy wooden door-frame strong enough to bear the weight of the condemned.
- Attach ropes to the lintel and hang the condemned upside down, by the ankles. *
- Obtain a blunt two-man saw from your local carpenter (teeth about 2" long).
- Sawing downwards, proceed until the condemned has been separated into two pieces, 1 leg each.
