quite a while ago:
Sitting in a low sec system ratting watching local like a hawk, look away from local fly over to a couple wrecks and in the brief 5 seconds i stop watching local a -6.6 Ishtar is on top and has already locked mwd'd over to me scrammed, webbed and released 5 x T2 beserkers on my poor little Omen  |

Was traveling several jumps in my pod. Was told in corp chat NOT to go through Rancer. I was too lazy to plot a course around it so I went through Rancer anyhow. A gate camp and a couple smart bombs later and I wake up in the station. |

had had my nightmare for about a week at this time, deadspace tank amarr navy tachs ts heat sinks everything but the rigs were pimped out, started to to a mission and was kind of tired, fell asleap at the kb.....woke up in a pod

I had one of those DOH!! moments when I was doing a mission in a Cerb with a remote repper raven by my side.
I took on the mob and had to warp out when I aggreed the whole lot of them (DOH!!)
Then we warped back in and tried again I aggressed the whole lot again (DOH DOH!!)but his time i had forgot to switch my mods on, bye bye lovely Cerb (DOH, DOH *******s DOH!!)

The idea of posting a topic like this dawned on me when one of my friend had a  moment today 
And like all things being discussed in EVE, it will eventually switch the focus to 'me' instead of the subject.
Why?
- because people can't read?
- that people're so full of themselves
- that ADBSD (attention deficit but self disorder) is common amoung EVE players?
- being ignored too much(either irl or ingame) and all you can think about is 'look at me me me! see what i have to say plzz"

When MY corp members shout out "FAIL!" it's normally me that did the failing. So telling you about myself failing is actually within what the OP asked for...

Well I day a friend asks if anyone wanted to help him run the blockaid mission with him for some fun in a 0.5 system. He was going to tank and the rest were to just be around shoting DED to let out thukker bretheren smuggle in relative peace after it was all done. I show up to the party with a fleet fitted Ferox one of my favorite ships. All is going well the DED battleships are dieing relativly slowly high resists across the board really annoy me but oh well. The 4th wave is there and i am remote repping the Machariel that is killing all the battleships. I lock and go to start killing the final battleship in the pack having just come into range. Target Lock Fire.
OMG WTF did I just do. I type into fleet "opps" right as the concord fleet shows up to kill me. I shot the mach instead of the DED and he was in a different corp. That was the end of my night missioning.

while in a FW gang recently, very late at night (about 5am ), am barely paying attention anymore as nothing has happened for ages... until a CVA t2 frig of some kind appears (malediction possily). As CVA being an alliance can't join FW, they're not a wartarget for the minmatar militia gang i'm in... so this CVA frig orbits the gang knowing if we shoot we'll take gate fire (us being in frigs etc, this is bad), and he can just warp off/jump.
My corp has CVA set with negative standings (red), so while everyone else is ignoring the frig, my half awake brain see's [RED], and deduces 'RED = BAD, KILL'... i decide to target him, to see if he bugs out. But i'd forgotten my weapons/scram were already primed. so once locked i opened fire on him (from 30km... not the 'ideal' range for small pulse w/multifrequency  ). very shortly after i'm in a pod.
Of course on voice comms i tried to bluff my way out with 'oh it's ok, i meant to do that... i'm tired and couldn't be bothered flying home. so killed myself for the pod express'. on top of it being a feeble bluff, to work i needed someone to pod me, and the FC (quite rightly) forbade it, saying he didn't want anyone getting 30sec aggression timer with the gate while in hostile space. So i flew home in my pod like a proper little spastic. 
Still too embaressed to log in and join another gang, as this is only about an hour after i spent time in corp-chat laughing at the militia noobs for being clueless. |

Well, I had a fun night, the other night. A 20 man(or so) group of neutrals came into a 0.0 that they didn't belong in. About 10 of the ships were nano vagas. I was in a pve fit drake raising my security status so I could return to empire, when I decided to join in some alliance fleet fun. I docked in a station, and quickly fit some random resists I had in a warehouse. Undocking, I proceeded to lock all 7 of my heavy launchers on to the nearest vagabond. Unfortunately for him, my missiles were hitting his ship for fairly good damage. Soon, his fleet realized he was about to die and 2 T2 Blackbirds began jamming me from 150km away. Funny enough, I got off one volley as the nanovaga's shields went down, and I watched as he exploded without me having a lock. The blackbirds continued jamming me for a while until another high priority target presented itself. As soon as they stopped jamming cycles another nanovaga began to take large damage from me. They resumed jamming, much faster this time, and also proceeded to call much of the fleet to attack me. Luckily, they saved my ship with their jamming cycles, as at 23 percent shields left, my aggression was up since I was jammed, and I was able to dock.
Hooray for a 300 mil ship kill, and the enemy saving my ship

A long time ago, I bought my very first battleship: a fully T1 fitted Armageddon, in Jita. I then went over to Motsu to try out a Level 4 in it.
A few days prior, I had split my two characters up into separate corps (I think one was in my player corp, and another was in an NPC corp or some such...)
While my salvage alt worked, I accidently pewed him with a beam laser. Not an hour after getting my very first battleship, CONCORD wtfPwned it ;_;. That was my "FAIL" moment of Eve. |

oh i remember another one, after a night of pirating in my missile rigged manticore and +4 set of implants and 3 5% hardwirings....i logged off with agro and you can guess what happened

Engaging with ammo in the guns but none in the hold, because I'd been futzing around cleaning cruft out of the hold.
Far too often for it to be funny any more And still I always assume it's lag or a bug until the realisation dawns...

Not a fail, more of a OMG...
I was can flipping in Haduango (or somewhere, .9 in minnie space) in a Rifter. I also had one of my corpmates tagging along to watch the fireworks.Anyway, I Came across a mining op with 2 Procurers, Hoarder an Osprey and a Rifter. All mining. They had unanchored GSC's out with no password on them (wtf...?). So I flip all 3 of them, and the Rifter engages almost immediately.
Just about instapopped...I had Hail, NOS and the guy had 2 civilian shield boosters for a tank. The Hoarder and pod warp off to the station, Pod comes back in a Reaper, Hoarder comes back in a Stabber - now its 2 cruisers vrs 1 rifter...But seeing as how badly fitted the Rifter was, I blew up the Reaper in their faces to see what they would do - Stabber engaged....and fires no missiles, so I just zip around him with my AB on and he misses every single shot
. Osprey engaged as well, was firing rockets at me, so I figured I could take out the Stabber if I stayed out of rocket range....NOS, scram, engage stabber, web Osprey. Couple minutes later, dead stabber, now its the Ospreys turn. Same thing, dead osprey. Now there was 4 wrecks in the belt, 3 pods, and one of my corpmates going "OMGOMGOMG" in corp chat.
tl:dr - bbq'd a Stabber, Rifter, Osprey and Reaper in a Rifter

hmm too many to list but here is my most recent:
We started FW and moved to Black Rise and I start running around the systems to check it out. Just got my rigged Nano-Zealot up and running and jumped in to a highsec system....Highsec PWNS -10 sec status. Yep Nubbins for not checking the map.

A few months ago, when I was with IAC, I was ratting in my megathron. Sniper fit with no armor rep, since I was lazy. I was also using heavy drones.
It works pretty well, until I let one frigate slip past me. Of course, I just HAD to let the one with the warp jammer come close.
So, I'm jammed. I sick the heavies on the frigate, but they can't hit him.
I watch, as it slowly eats through my shields, as I hope for my heavy drones to start landing hits. Going into armor now.
80% armor.
"Should I call for help? No. I can kill them on my own. They'll start hitting the frigate."
70% armor.
"Well. ****. And my friend just logged."
60% armor.
"Dammit, I'm not going to put up my killmail saying that my Megathron was killed by a ******* frigate."
local chat: Er... Can someone give me a hand? I need help killing a frigate that's got me jammed in belt V-1
You need help killing a frigate? what are you flying?

I can't. I've been ratting with a sniper setup.
