It will not be possible to get perfect refines anywhere after the Crius patch (the refining changes have been moved out of Kronos to the next major patch, scheduled for deployment on... 22nd July I think?).
That being said, CCP are buffing the raw mineral content of all ores and ices also as part of Crius, so it will be possible to get the same amount of minerals/ice products from refining as you currently do with a perfect refine, even in hisec. To get this, you will need Refining 5, Refinery Efficiency 5 and the relevant ore processing skill at 5 and a Zainou 'Beancounter' Refining RX-804 implant. With all of those together, you will get the same amount per m3 refined in a hisec station as you do from a current perfect refine.
Note that it is going to be possible to improve on that, however - Crius' changes will give hisec stations a base of 50% refining efficiency, but it will also change the efficiencies on POS refining arrays, with one anchorable anywhere (including hisec) with a base of 52% and another anchorable outside hisec with a base of 54%. Player-built Outpost stations in Nullsec will have the 50% base the same as hisec, but their refining can be upgraded; for Minmatar stations this allows a maximum of 60% base efficiency, while other stations will have a maximum of 57%. All of these refineries will benefit from skills and implants to improve those base amounts.

No, you don't. Because the industry changes have been pushed back to Crius, so post-Kronos you can do with your current skills.
Post-Crius however, you need Ice reprocessing V, the implant and the standing to get to where were before.

So what is current formula now, after Kronos?
I get: 92.4%
I am refining at a station with, Base Yield: 50%
My skills are:
Refining: 5 Refinery Efficiency: 3 Ice Harvesting: 3
No implants. But I would like to know the formula so I know which one to start with and if implant is worth the trouble and price.

Your question is: what skills/implants will be required for the most competitive Ice refine at place X? You'll need to read the feedback thread in order to look at pilot answers to that question. But then, it won't matter because CCP will change it 3 times before they release, and then it will be broken. Best to wait until the patch following the patch following the patch before taking a chance on getting a solid answer
