Yeah okay so that other thread got me thinking. I have only really gotten near heart-attacks from 3 games this millenia.. But all in a different way, which is sort of funny.
Dead Space - What a ****ed up game, really. The heart-attack scenes here include cheap scaretactics like "oh i know something is gonna be behind that corner, listen to the music, oh wait, its empty, ok then, next corner, they are just trying to trick me... Oh okay, nothing. K i'll just save the game" *BOOM* a friggin baby with tentacles jumps out right in front of the damn savespot!
Street Fighter IV - Pretty simple stuff, online fights. Thats it. Every single match i feel like my heart is about to collapse in on itself lol, even if i dont really care if i win or loose. For some reason i didnt get this rush from the old SF games, but this one is aging my heart unaturally fast.
EVE pvp - The worst heartattack there is.. When i'm shipping frigates through 0.0, and i get locked a split second before i jump, but manage to warp away due to being a few meters outside scramble-range, my heart is beating like an *******! And dont get me started on fleet or 1on1 pvp, my mouth starts drooling and i curl up on my chair like a monkey, fingers tapping on the keyboard and the capillaries in my eyes bursting.

Cryostasis, only been playing this game for just over a week but some of the moments where you come out of a flashback just make me jump out of my skin. It's the first game to make me fall out of my chair... other than when I knocked myself out from drinking Coca Cola (greatest achievement in my life).

talk of Doom 3 reminded me of playing the original Doom in a darkened room on my old 486...
Those Imps could actually be pretty scary - you could hear them from a way off but they could still appear behind a door and start trying to rip your face off as soon as you opened it...
Quake 2 Multiplayer DM and LM CTF were the only other games to give me a real adrenaline rush since ===================================== It smells of spoon! ===================================== |

I don't get that from anything anymore, sucks.

The game was a typical sword clashing FPS but at times when nothing was happening and you were running around trying to find the protal to get to the next level, it got boring. Suddenly, a ghoslty face would jump out of the screen screaming at you, similar to those jokes on the web. You can never get used to it, the designers must have researched on concentration times because it ALWAYS happened when your concentration started to lapse.
I used to like playing those games in the dark for max effect but I had to turn on the light and walk away from the PC after a three hour stint of playing. I was still shaking an hour later and looking around doorways cautiously before entering the kitchen and such.
I have to say it was the most nerve wrecking game I have ever played, great fun but absolute hell on the nervous system.

The cradle had to be the best part of the game but Thief always managed to make me jump a few times. Hide in a dark corned as a guard passes right in front of you, your shoulders drop as he passes and you think you're safe. Suddenly, "Ho, Taffer" and you realise the guard saw you as he walked past but it always manages to catch you unaware.

Bioshock - In some dentists office that fills up with steam so you can't see...the steam goes away and it looks like all's clear...you access the booster station and when you're done you turn around and AHHHH there's the creepy mutant dentist 2 inches from your face......instant ****-in-pants. That one got me bad the first time.
or the statues in the flooded basement....turn around and it's right behind you, then comes alive and attacks....booga wooga.
And I seem to remember getting creeped out in the original Unreal game, not the tournament ones. Something about hearing your buddies torn to shreds behind a door...don't remember too well, must have fried that braincell.
And yes, Doom 3 was endless jump-out-of-pants moments...

I must say I was horribly disappointed with FEAR 2. The first one had me rocking back and forth crying "mommy" for some scenes, but at least the demo of FEAR 2 felt so...I dunno, predictable. Nothing really surprised you and as such I was really scared. Also, first time in Ravenholm and the FIRST Alien versus Predator game. Last bit of jumpiness I got was actually an older game. Vampire, the Masquerade. The level where you enter the haunted house.
I was completely not expecting to get hit with a d*mn picture frame.

The demo for FEAR2 is bits from several levels clumped together into quite a short level so the pacing is a bit "off".

This thread annoyed me i thought it was about a real heart attack like some num nuts sat up for days on caffine and his heart exploded like that korean matey in that internet cafe

You KNOW that something is going to happen, you can hear the necromorphs in the vents [even worse on headphones!] the music picks up, the lights go out and something screams and a little poo seeps out and something seriously ****ed up comes running at you, flailing and screeching horribly. You shoot it in the legs, realise something's behind you so turn around to see another necromorph burst out of the vents and come at you. Empty an entire pulse rifle clip into it and then turn around again, only for the first necro to be slowly crawling towards you, arms wildly spasming everywhere . . . you shoot it, walk around a corner and suddenly that corpse you just passed over jumps up and ****s your brain
I can't play Dead Space for more than 45 minutes at a time, because the one time I tried it I kept on hearing things and looking over my shoulder for the duration of the next day!
NOT A GAME TO BE PLAYED AT NIGHT
Also, I have to agree with Halo. The first flood level made me drop a brick or two simply through pure tension. -----

I have had 2 heart attacks in RL... trust me it's not a good way of getting 6 weeks off work.
I now try to avoid PvP in eve, because although it's only pixels, for some reason it is the most adrenaline fuelled PvP in any game I have ever encountered.
The only other game that made me jump on occasion was Half-Life the first time I played it.

And of course Thief I and Thief II, especially on hard mode. I held my own breath back some times when sneaking in those games, only to jump up from the chair when a guard started yelling behind me. Good times:)
System schock deserves a mentioning, but could never get me that horrified/on the edge. And in Bioschock, death was only an annoyance, nothing to be feared, so it took the whole fear element from the game.

Dead space, reasons already listed in this thread
the S.T.A.L.K.E.R games, most of them arent THAT scary, reminded me of walking through a horror game after someone else cleared out most the scary things, but, some of the underground lab complexes... WHAT..THE..*****
nothing gets me a bigger adrenaline rush then high stakes pvp in eve though take out a ship that you cant just comfortably buy 5 of, like faction ships for example, it's not that scary when you are just risking a simple frig/crusier or something --- return the mines!

clive barkers jerico is also pretty scary, and good atmosphere, sadly, the gameplay is somewhat lacking the main thing that kept me through it was the curiosity of what freaky **** could possibly be behind the next corner
now that i have finished it, i'm pretty sure i will never install it again though --- return the mines!

Try out undying then. It had a lot of freaky scenes.

i havent played a game or seen a movie in so long that actually made me feel scared.
I remember the feeling of playing certain games with all lights off at like 3 am and scaring the hell out of myself and i loved it. Its a feeling of total immersion into the game your playing.
Im still hoping for the day ill find a game that sucks me in enough to make me scared. Its like your first high on a drug. Youll always be trying to achieve the feeling you had the first time.

it's not exactly the sex he's talking about, he's talking about that if you have a bad heart, not being in a good shape can make it worse
i however like pointing out that being in too good a shape is also bad, the wierdos that run a marathon every day have a nice chance of getting a heart attack before 40, even if they were born with a good heart :) --- return the mines!

I dunno, I'd consider myself to be in better shape then most and I still get mini adrenaline attacks whenever I get into a scrap, or whenever I jump in on top of a mining op. Left 4 Dead is another game that really gets my heart pounding, but in a different way. With EVE, it's because I know I have something to lose when my expensive ship and implants fly into a fight. With Left 4 Dead, it's because I'm literally being smothered with zombies, something that I'm rather phobic of.
In either case, I'm not sure which I like better, but EVE's PVP has always kept me coming back for more.

Alien vs predator with colonial marine and director's cut difficulty takes the heart attack prize for me. Fast paced combat where you and your enemies can die from a single hit, aliens are lightning fast and can move on all surfaces, infinite enemies, finite ammo and health, dark everywhere and YOU CAN'T SAVE YOUR GAME.
Basicly you wanted to stay in easy to defend positions and calm down, but you knew, that if you stopped moving forward, you were just lowering your chances to succeed. It actually worked well, since the missions weren't that hard, if you kept your cool, kept moving constantly and shot accurately. The whole game was just designed in a way, that made those simple things hard to do.
Playing it when it first came out in the middle of the night, is still the best addrenaline rush I've ever had. Add to that the facehuggers. One shot kills as it jumps on your face and inserts its "thing" in your throat. You could hear the clicking sound the facehuggers made when running on the metal floor and you started to shoot wildly at all movement.
Good times.

I always thought an MMORPG based on the HP Lovecraft mythology would be kind of cool, especially if it wasnt all about weapons.

Player retention would be a problem though, since the basic concept is that everyone get eaten or go insane in short order… It just wouldn't be done right unless you lose 2-3 characters per login.

I agree about the player retention being a problem, and its in a very niche market place, really aimed at Lovecraft and CoC fans (When RPG's where real RPG's!!).

Yep, was dark and scary, its the only game in 25 years of gaming I have never finished because of that
Fear, Doom 3, Half Life (Ravenholm) were a breeze compared to it, always fancied giving System Shock 2 a go though ... not got around to it.

Dear christ god jesus, I thought I was gonna die on every "triangulation" from the helmet computer, because you KNEW aliens were coming, from the floor, the ceiling, the lamps, the doors, your back, EVERYWHERE, and the sound of the changing view-modes, and the constant, CONSTANT "heart beating" of the predator. Everything about that game is so unbelievably atmospheric. It's the ONLY game, really, the one and only game that has forced me actually force-quit the game, shut down my computer, and go to my bed to literally curl up below my sheets and cover my head with my pillow, and I mean, I literally did this, because I couldn't take it anymore.
EDIT: I HAD to go look up a video for those that haven't seen or played this game. Look at this, and notice the little sounds, the atmosphere, the suspense. Now imagine this at 4 am, alone. Now imagine you've been scared/fascinated with predators all your life (it's a love/hate thing for me).
Really, I can't stress this enough, if you want to play one of the best games ever made, go get this now, but the first one, AvP1.
Just looking at that video made my heart start racing.

I will explain a few rush-y moments before I go into the times that I was scared of playing a video game:
1. Gears of War: When I first played this game, it was awesome. Problem was that I had little sisters in the house, who absolutely COULD NOT be anywhere near the TV when I was playing. One night, I had just gotten to the level with the Berserker for the first time. The scream that it makes as it charges you... at 2:30 in the morning, with NO lights turned on...when you are the only one awake..
The second point where I had the **** scared out of me playing Gears of War was the level where you have to sneak around the lambent(glowing, exploding) ground-crawly thingies in the lab place. I could hear them running around and making noise, but then I kept falling into their little pit thing and had to keep scrambling for the ladder, lest I get ripped to shreds.
2. Mass Effect: I loved this game, and it never really scared me, but some of the combat game me rushes, like when you are shooting at the more intelligent AI. Long battles with enemies that sneak around makes me hate my adrenaline glands.
3. Army of Two: Playing on the hardest difficulty on the airport level. I had picked relatively light weapons and ended up running out of ammo for my combat shotgun. Eventually I ended up going on a melee with the Desert Eagle with a tiny amount of health. It was insane
Finally...
4. F.E.A.R. for X360: I never did beat this game, it scared the **** out of me so bad. I had terribly low health and low ammo when I got to the second level( the office building.) When the level starts and all the enemies come POURING out of the building, you have low health (like 40/115) and you have guns that are low on ammo AND ill-suited for the terrain, it was scarier than ****. I died so many times. I got partway through the level and that's when I knew the ninja-things were coming. I could see the ceiling panels falling, the glass breaking, and decided to call it quits for the night after that.. I was playing with all the lights on, everyone was in the house, awake, and I had the stereo turned up pretty loud(surround sound.) My dad never let me live down the fact that I yelled so loud that he heard me from the back room, 40 feet away.
5. F.E.A.R. : the Flashbacks. These scared the **** out of me so bad that I eventually had to start plugging my ears and closing my eyes when it happened. Going from normal gameplay to a flashback nearly induced a heart attack every time, especially when a skeleton came at me, or Alma decided to show up, or the screen went to a "normal" flashback. Then, nothing is worse than coming OUT of a flashback, and getting mobbed by heavy soldiers in the armor plates, when all you have is the machine pistol.

The most recent game to get my heart pumping was Left 4 Dead. It's not particularly horrific (although very gory), but some of the suspense can really get you going, especially due to the fact that zombie placement is different every time you play it, so you can't get used to patterns.
Before then, the last one would be Bioshock. That had such a creeping atmosphere of dread that once something happened, you'd react much worse than you'd normally would. It's a great example of building tension.
The original FEAR, Aliens Vs Predator and Doom 3 were all great examples of games that made me poo myself at some point. Many critics did pan Doom 3, and I can understand why, as it is bog-standard run and gun one man army stuff and offered absolutely nothing new. However, it did have buckets of atmosphere and worked very, very well at that. FEAR just got me with little moments like when Alma appears fleetingly as the lights flicker on and off. Aliens Vs Predator just had that motion detector, which is a wonderful device for building tension.
There aren't many games that do this to me, as there is such a reliance upon gore to make up for the lack of tension, much like most modern horror films. Where these games have succeeded is that they were well-constructed so the atmosphere builds up the tension before the pay-off.
EVE is a different kettle of fish. I've had those adrenaline rushes when running into an unexpected gatecamp whilst in something valuable, or trying to flee a massive hotdrop in a slow-as-****e battleship, but I think I've become much more accustomed to this since I got into the regular PvP thing (although it still happens occasionally).

I never really found the original FEAR to be that scary. Too freakin' predictable - I stopped playing for ages as I just got bored with it.
FEAR2 sounds fun though.
I've officially shat a brick on Deadspace and HL2EP2 with the Hunter at the start. Goddamn.

EVE PVP gets the heart racing though - I know what the OP suggests... Adrenaline rush. Not even sure why, get it in CSS sometimes as well. Heart feels like its trying to get into my windpipe.
