That is, does the number on the wormhole show the properties of the wormhole itself, or the wormhole system it leads to?

The properties of the wormhole. (Of course, one of the properties of the wormhole is what class of system it leads to.)

I've been away from EVE for awhile, I'm not sure if they've kept it up-to-date or if there's any better ones out there by now.

each class of womrhole has a code any connecting holes will match up on one of about 8 different types depending on k space or other wspace
3 classes of k ( hs, low sec and null) 6 classes of wormhole THere are about 50-60 different codes for all of the above

Wormhole codes (X5344) Will ALWAYS lead to a CLASS 5 wormhole Have 2.5 Billion Mass alowance Have 24 hours total time
Mind you there are 100+ Class 5's to choose from, so No, its not always gona lead to the SAME class 5.
Obviously the above example is made up. But illustrates the point i think.

But, within a class, will the entrance WH always be the same?
For example. C125 leads to a class 2 wormhole system and can handle 20Mkg at once. D364 also leads to a class 2 wormhole system and can handle 300 mkg at once.
Lets say you go through a C125 wormhole to a given system. Could a D364 wormhole open up to that same WH system? Or would only other C125 wormholes lead to that system?

simply .. NO
If you scan out a WH with code X702 and your friend/alt in a different system scan out a WH with X702 as name and you both jump through it, you will arrive in say J123123 and he can end up in J234234, there is ofcourse the random possibility that the WH you jump in have more K-space entrances (i believe up to 8 WH's max at a time to connect to a w-space system), but that possibility is slim as there are so many systems in wspace.

all the answers are correct each class of womrhole seems to have 6 other possible destinations ( high sec low sec 00 and then 3 other types of wh)

yep, think so :P we have a "static" always the same name wh, and sometimes inbound whs from other nametags.

class1 only lets specific ships in. others are mass restricted, youre usually fine up to Orca (jump in AND out 3x).
And there is a nice forum thread that shows you what each Label means and where it leads ;P
