What are the different types of payment plans you can make with merc corps?
It seems that one way corps do this is to pay a certain percent of isk destroyed. How does insurance factor into this? Lets say Corp A hires merc Corp M to dec Corp B, for say 40% of isk destroyed. Well if corp B keeps coming out in fully insured t1 battleships it would seem Corp B could pretty easilly bankrupt Corp A. Or do they calculate isk assuming platinum insurance?

I can only speak to how we do pricing as varies alot from merc to merc.
However a kill contract is based on hull cost ussually calculated at current market prices. Insurance isn't accounted for as it's a metric for payment, a way for us to produce a desired result and not a measure of isk damage to the target. We don't count mods lost in the price which is often a large part of the player loss.
We also set a cap if requested, you tell us your upper limit and when we hit that we ask for more or stop. That way you don't end up with a bill you can't afford.
You can always negotiate a flat fee contract for the required time frame as well eliminating the kill calculations but if the mercs don't kill alot it's mite expensive if they do well it's cheaper.

While most groups have their own pricing plan, they are almost always open to other options. Personally, I prefer to pay War Dec Fee + Pay Per Kill, with a flat fee based on ship type. This means that, should some idiot in the target corporation put on some X-Type modules on his Ibis and gets popped, my budget is not gone with that one kill.
Also, a flat fee per kill helps in that T1 ships are insurable, and the target doesn't lose 140m isk when their Abaddon hull pops. Kill boards are also notoriously horrible in their accuracy and some groups will modify the price of items to ensure that you pay a premium. In addition, the merc groups get the loot anyway, which is an added bonus.
Some will work for a flat fee per week, but I warn that you should only hire well known, well respected mercenary corporations under this kind of contract. Some mercs will get the money and say, "Gee, you know, we had a war dec for a week but no kills, sorry, thanks for the cash!"

Pay me 5M and I'll tell you how I price contracts.
But as for the above, yes, a flat rate is usually good for trusted mercs (wait...InB4Noir!) but, if you don't trust someone (like a good capsuleer), then pay-per-kill usually works well.

[enough ISK to make one merc happy] x [merc force size] x [merc corp reputation] = fee.
