Happened to me yesterday.
Hopped on a thorax and just put on 2 hardners appropriate to the intel of enemy ships I had.... everything showed just fine and dandy.
Hopped a couple of systems and went towards a gate I didn't have instajump for.... tried to enable mwd but I got:"[ 2005.01.02 07:48:52 ] (notify) Unable to activate Speed Boost, 10MN MicroWarpdrive I is not online."
Weird... it showed online just fine... by rightclicking it even gave an option to put it offline... checked powergrid and it was as it should have been with the modules I had.
I clicked to put it offline , waited to get 95% cap and I tried 3 times to enable it but with no luck... nothing was happening and was still getting an option to put it online. 
Suffice to say and after my partner was engaged by a nearby gate the ship was lost because nothing other than the hardners I had just put on worked....guns,web,scrambler,armor repairer all seemed online just like the mwd did and all gave me a similar not-online message.
Now I could care less about a thorax, but does this mean I will have to put every module offline and online again before leaving the station just to be on the safe side? 
Petition ofcourse got me the standard template response but that was expected given their... investigating tools
Has this happened to anyone else?

i had heard that if you logoff in space all your modules are turned off, however if u came from a station, then it looks like a bug O.o

No it was in normal space and the thorax was normaly parked in a station.
I activated it, took off some caprelays and put 2 hardners on and exited the station like any other day
I think I would have noticed if ALL my modules were offline since I visited the fitting screen lol...
Even if I was dumb enought not to notice, all of them showed online in the in-space GUI and I got a screenshot of them as well. (but ofcourse player submited evidence can't be accepted because screenshots can be tampered... I don't know why we even bother sometimes)

So there was I one day happily hunting rats in my Dominix. I warp around the belts until I find me some small cruiser and interceptor rats. As I proceed to kill them I notice with horror that I'm taking heavy damage. I check my repairers only to find that all of my low slot modules were offline
I've managed to kill off the interceptors and warp out in time, but CCP still owes me a fresh set of pants.

Mine was very different though.
It's not a case of "oh damn they turned offline while switching/boarding ships" which happens a lot.
Mine appeared online, I could have the option to put them offline, but when I tried to use them, I got a message that they weren't online.
The weirdest thing is that when I clicked for the mwd to be put offline, it turned grey like it should but wouldn't come online after that even when I had over 95% cap

I had a similar bug, but quite a long time ago that seemed to be related to the use of multiple micro aux power cores (how I miss dual mwd vigils ). It seemed that when I undocked, everything would get recalculated, sometimes resulting in Eve getting confused about just how much grid I have, and therefore treating some modules as if they were offline (but showing them as online).
I don't suppose you have more than one module that raises grid fitted do you ?

I had this exact problem happen to me just the other day in a Tristan. Undocked my ship, went toward a gate, tried to fire up the afterburner and it gave me the same message as ElCoCo, afterburner is not online. Waited for 95% cap and tried again, nothing. Went to a base, removed everything fitted in my ship, including the afterburner, and somehow something was still taking up CPU and power. The only way I could get my lost power back was to use the strip all fittings button. I repeated this several times and eventually just got sick of it and removed the afterburner for good.
Oh, and I tried asking on the help channel about this, nobody knew what to do about it. Is there a workaround?

Had this bug a few times, on both taranis and thorax, but not for months now. Out of curiosity is your setup only fitting because of advanced weapon upgrades?
