(There are no tears in this thread - I apologise for those who are thirsting and suggest they lick one of the many other such posts in this forum) 
My son (yes, REALLY, not me! ) returned to EvE recently and lost a ship last night to a looter in a deadspace mission (0.8 - no accel gate). I'm just trying to figure out what he did wrong (no - accepting a hisec mission does NOT count!) so I can educate him when he gets home later.
He says the guy flew in (scanned him out obv.) and looted stuff, flagging himself in the process. Then targetted my son, who had no drones out, and was ignored. Son continued with mission, killed all drones, then realised he was still being damaged and saw "'PlayerX' (no names here) hit you for XXX damage" type of message, saw the damage, so responded - and lost as he was scrammed and couldn't get away in a mission fit ship.
My best guess is that the looter jetted something out, flipping the loot can, and maybe Son opened that can after the looter was already flagged to him, otherwise Concord would have intervened. (If - as he claims - he didn't do a Han Solo...)*
Anything else I might be missing that could have flagged him back?
(If he wasn't flagged back I don't see any way the looter could have shot him without Concord interference, but I don't run missions much so I may be missing something here.)

I would suggest he shot first whether he noticed or not. Check if he has "auto target back" turned on, as he may not of noticed if his ship auto targeted him and just engaged the guns. Otherwise your suggestion of him droping an item is in essence good in theory but it does tell you when your stealing from someone. Hey may have ignored the warning but one would hope he would have thought more about the random prompt he recieved.
I suggest he just shot him by accident. But hey, atleast he didn't loose anything cool like a fleet or pirate issue battle ship eh!

If he actually did NOT shoot first, either accidentally or via auto-lockback asmentioned above, then perhaps his drones were set to Aggressive? I have seen that happen where a wayward drone decides to take a shot at the flagged player.

The Auto-lock thing is the most likely candidate. Something lots of people are guilty of when flying without due care and attention, or just because they don't know it can be turned off - and to be fair it does make missions easier if you just select the next thing in that nice little queue displayed across the top of the screen that you've already target-locked, and hit "fire". Problem is, if your son had "auto-lock back" turned on, then by locking your son the ninja just added himself into that queue.
In the [ESC] menu, goto "General Settings" and look in the centre column. There is the option "Auto target back" then a scroll box with anything from 0 to 12 targets.

Sure he took damage or just see the message? Make sure the ninja didn't warp scram a can named "you". Been known to happen.

Auto-lock is my guess. Even if you have this disabled, trying not to lock a ninja that has turned red requires being very careful when you still have dozens of red NPC ships to kill. It happened to me last week. I had to be careful of what I locked when this ninja appeared in the mission and turned red.
I also suspect that AFK missioners get killed a lot by setting their drones to aggressive and walking away. Couldn't their drones attack the first ninja that turns red by flipping a can or something? ____________________

Dude...
Simply take a look at the relevant game log. Look in <My Documents>/Eve/logs/Gamelogs, find the log around the time of that fight (they're named YYYYMMDD_hhmmss.txt), and take a look.
You will see exactly who fired first.

OP, 9 times out of 10, we loot and get shot by the MR. No rocket science going on there and Occam's Razor would suggest that's exactly what happened. Occasionally, someone will loot from one of the cans that we throw out and then, at least for Suddenly Ninjas, it's a party. There is a very small chance that your boy looted from one of his own cans just at the same moment that the ninja was doing so. This is dicey and doesn't work all the time, but it does happen. Another possibility is that the ninja flipped the mission OBJ can and your son did not notice it as he was too busy fending off those pesky strain alvi warp-scramming frigs.
