Wow.. I was really hoping no one would respond to this lame topic.. but I guess people just have to be 'first'...
This issue is soooo easy to duplicate, if anyone remembers the portraits that showed the teeth and eye balls as a solid black color and the rest of the head was gone, its back and can be easily repeatable.
When doing your portrait, goto settings and turn shadows to low, let load and then change back to high. When its loading your portrait, press the 'camera' button to get a snap shot and bam.. its all f'ed up. (It would take a few seconds for the correct rendering/lighting to affect the model. Not sure if this works on all machines however.)
When are these dumb 'how do i look', 'how to make character avatars', 'make boobs', 'blah blah blah' go away.. Sick of reading all these useless topics in General..
Hurry up with the next 'fad'.. sick of reading about this one already!
CCP - Make a Channel for all these people to go, EVE Communication Center -> Character Creation. That'll fix my headache :)

I had a bug where the background was visible underneath the collar of my jacket... kinda cramped my poses.
