Without trying to shed tears here, as my trust in CCP to make something good has eroded over the years I'm trying to find a new MMO.
- doesn't need to have spaceships - must have meaningful PVP - must be "epic" in the way that's it's actually worth my time and money - can't be WOW, darkfall or Rift - can't be a "Korean" grindfest - preferably new-ish so the game/players/devs aren't jaded yet - can't be vapourware
Any ideas?

Wait for either Tera Online or The Secret World.
The first is Korean, though not really like others, look it up. The second... Well, look it up too ;p

APB. It's free, no storyline to follow, and epic battles happen every second.

perpetuum online is an eve clone with robots and best of all, CCP doesn't develop it. Might be worth a shot, I beta tested it and it was equal to or better than Eve. Smaller universe, but smaller population so might be worth a look see.

Check out the "Free to Play" section on Steam.
I can recommend Forsaken World if you want a fantasy setting. Global Agenda's not too bad if you want something more sci-fi.

RL is cool atm, perfect for beginners and bitter vets, doesnt matter if you are into PvE or PvP you will always have something to do, there isnt any lag or DT in RL but weekends can be a pain in the arse.
I would advise paying attention to your health though, having bad health will seriously affect your ability to PvE, I dont know much about PvP since im bit of a carebear but I heard Libya and Syria are contested, you could claim them but you will need to NAP NATO to get anything done.

You can give LOTRO a try. It's free to play.
Game's lore is as noted by the LotR IP wonderful and gameplay is fun and ez to hop into. Have 2 65's there and still enjoying it since 2008, community is wonderful and you will find the higher level you are people are nicer and more helpful. Even me, with my bitter vet jaded attitute here, I must confess when I log on there I help people, something about the world I think. I'm on Brandywine server if you want to give it a try, you can contact me through STEAM.
Talking of Steam, it recently launched a section of F2P titles too, and steam's shell is perfect for gaming, also it's my main IM /Voice client too, give it a try, some interesting titles out there, and some people seem to enjoy Forsaken World.
If you want a nostalgia trip or want to check out how MMO gaming was back in the day, you can try EQ1, they launched a couple months ago a progression timelocked server for nostalgia purposes, that is slowly unlocking all expansions since original EQ1, I had a blast there for 2 months, since I'm an old gamer, re enjoying my Necromancer leveling. They are one of the oldest mainstream MMO's around, open since 1999.
Eq2 is pretty rock solid, having been around for a long while and with tons and tons of content with the same IP of EQ. Also up to this day NO OTHER MMO has a damn fine housing system as theirs, its simply the best housing system on any MMO I have played.
Good to see you skipping the crap that is RIFT, it's practically WOW 2.0, I totally hated it, thanx goodness client was a gift from a friend that owed me bet money. I didnt waste any real money on that piece of stoma excreted filth.
If you want more new stuff, World of Tanks, lots of eve Players around so plenty of o/ and o7 in there, if you want something WWIIish there's even a Company of Heroes online if you like rts with heroes that level up in an online world.
Also as a last solid choice, try FALLEN EARTH, like EVE it's a single shard universe, set in a post apocalyptic wasteland. It has GUNS and MELEE weapons, as well as PVP free for all areas in the world PLUS arena like matches in Blood Sports. I was an avid follower of Fallen Earth until real life, moving, etc made me stop playing. Would love to go back but between LOTRO/EVE I honestly dont have much time for anything else, but give it a shot, you might love it or hate it, its one of those games. Also the Wasteland is HUUUGE and without a map or opening the ingame one you can get lost in the barren wasteland, thats what I liked the most, hopping on my bike, machetes close at hand, scavenging the trash of past generations eking out a living and helping people solve their problems as I made money for fuel so I could keep wandering. Some people , the rich ones have cars, or sand buggys.
Game is relatively new, it launched in 2009 if I recall, has had it ups and downs, might even go F2P soon, so pop will swell up.
Seems EVE is going F2P soon the way they taking it , meh.
So TLDR?
LOTRO or FALLEN EARTH Cheers!

Anywho, there aren't any decent MMO's out right now aside from Eve. Honestly, you think you have it bad here? Try another MMO for 2 weeks. You'll be back soon after.

"Smaller" is a gross understatement. When less than 300 people log in at peak, there is something wrong. Yeah I am a pirate. What are you gonna do about it? Killboard link is not allowed to be used in a signature.Applebabe

You might be surprised or even call me stupid, but Trek Online is REALLY starting to shape up! Hilariously dorky Vidcast on the coming attractions.
- HAS spaceships. - PVP not that meaningful, granted. But there¦s space & ground PVP! So it¦s there and also up for ACTUAL iteration. - Pretty epic, especially if you¦re a trekkie. - Devs are pretty dedicated and even the new boss fights for the special pay to play ships becoming available through in game means (aka grind, sorry).
I¦d put it on the radar when their season 4 comes in.

I like how they talk about wanting to create characters and mold them like we do in pen and paper. Also, I like the idea that one will have to protect and attack in all three dimensions and players will have to protect against attacks coming from above.

That looks like it could have some potential.

Try APB - all points bulletin. It's in beta now, but registering and getting an account takes 15 minutes tops.
Awesome pvp, usually battles are no more than 4v4 but since there are 100 players on each map, it seems like everyone is shooting at you.
No leveling, you have to grind standing to agents, but your character has no such things as levels. Thanks to standing to a certain person he gives you some nice stuff to buy.
Fully customizable items like cars and clothes, you can paint them all the ways you like and also you can buy painted by others. Not to mention upgrades.
In general, even without paying cash to anyone, you stand a chance against players who spent months playing the game, so it made me stay for a longer time there.
Most important - it's fun, f2p and it's an anazming time consumer!

It's fun, pure PvP and highly addictive.

Just gave Perpetuum an hour or two of my time, definitely going to give this more time to see how it is.
Installed APB Reloaded last night, still in tutorial area there but it seems like some good mindless mission-driven pvp fun so far.
Going to also give WoT a whirl tonight as well since most of our alliance seems to play it (and/or Minecraft which I still can't figure out what the point of it is lol).
Kind of excited about Battlefield 3 as well after watching all the E3 bits and such. I've not really been interested in FPS games for a long time, but BF3 will have 64-player maps for PC, and it does look fairly righteous.
I suppose none of these except maybe Perpetuum fits your requirements, but who knows, you might give one or all a try and find some solace ;)
PS as much as it pains me, I'm truly thinking about trying WoW for the first time since my wife has given up on Eve and started playing it. I'm down with Diablo-ish loot-fest mindless fun, but mostly the fact that my wife plays it means we back to gaming together in a group like we always do in Eve. Stuff goes here |

If you want something pvp-centric, I'd recommend League of Legends. Small battles (5v5 is the biggest, I think) and lots of micro, but it's a good bit of action.
World of Tanks has 15 v 15 tank battles. Start with a tank that was outdated by WW1 and move on up to late WW2 tanks / prototypes. Some frustrations with balance there, but overall a decent game.
Forsaken World is a Chinese-based game, but made for a Western audience, which means minimal grinding (questing is how you get your EXP). I like it enough so far. It might be a good enough time waster until I see for sure how some upcoming games look after release. It is a pve-centric game, though, from the looks of it. I'm not sure how pvp on it works. They have open world pvp and arenas (3v3 and 6v6) so far as I know.
Can't really speak for any other games. You don't want WoW or Rift, so eh. To be honest, other than Eve Online, I'm not sure what else has "meaningful" PvP.
