I've been in plenty of ops where there has been people shooting at each other and so on despite the commander saying stop messing about or i'll pod you.
Usually if the commander is fecked off enough, the offender has his sheilds and armour stripped off, and suitably chastised he doesn't do it again.
To the guy killed: Lesson learnt. Don't mess about on ops when you are told not to.

Teasing your superiors, like CEO's and commanders are fun  .. up until certain point where you should REALLY stop it. I got yelled few times last weekend on mining op for webbing haulers out of boredom and launching drones after em.. almost killing one hauler

Look at it this way, could always be worse. When I was in FA, we had a guy who kept shooting one of our members, which got annoying, but when he shot the vice-CEO only once, we not only blew up his uninsured raven, but podkilled him about 3 times before he got to the cloning station safely, iirc...

he was just beating up poor ickle pnyx coz he was in a vigil! ABUSE OF CEO RIGHTS!
Althought, its not as bad as the time he killed 2 of our battleships in a gankageddon before he realised they were friendly (he got confused, first day of shiva :()
