The basic relationship between these two entities has eluded me. When I look at the Market Details of an item - which lists a variety of buy and sell orders - are those NPC orders, or PC orders, or both?
Is an item sold into Escrow added to this list?

No relation between the two.
Things on the market are sold by NPC's or by players, majority players. You can see who's sold you anything by looking at the transactions tab in your Wallet.
Things on Escrow are sold by players only. Initially the market could only sell non-named Tech 1 items (example: 125 mm Railgun 1) and not the named Tech 1 or Tech 2 (example: 125 mm Gauss Rail 1). So, escrow was used to sell named modules, as well as blueprint copies and anything else.
Recently, though, they've expanded the market functionality to include these named and T2 items, and so the only thing the Escrow system has that the market doesn't is Blueprint Copies.
Otherwise, Escrow is generally visible from any region, and you can put anything up for sale there. But it's so crowded that things don't stay listed for more than 6 hours, and it's also prone to scams. Market, on the other hand, you can only see one region at a time.

Thanks for the info. How does one put an item for sale on the Market? The Buy Order button is obvious. Do I need a special skill to put in a Sell Order?

Right click the item -> sell this item. Choose the Advanced button in the window that pops up, and you can specify price, for how long it should stay on sale, etc.
You don't need any special skills, but you're limited to 5 orders max. The skills under Trade (specifically, Trade and Retail) increase this limit. The other skills in there deal with remote sell orders (from the assets window) and/or reducing the taxes.
