Most embarrasing thing I ever did was probably when I had gang leadership of a group of players who banded together to defend fountain core from evil invaders (prolly bob or atuk/shinra). I was warping us around between gates in the hope of catching something to shoot, when we find a hostile pod at the 75fa gate in yz-lql. We try to engage it, but it warps away. I try to see where it warps, and yes! It warped to a nearby asteroid belt. Eager to catch it, I gang-warp the entire fleet to that belt, at 15 km. I mean, who wouldn't do the same to catch a pod? :D Right, so the pod warps away of course, and the npcs attack, and I try to gangwarp us away from there, after telling everyone to get out of there of course, completely screwing up any aligning the guys had been doing. They all get out, one rax in structure, and I stay behind to watch them all leave safely, so I lose my thrasher.
You can imagine I was pretty embarrassed that day, not for losing the destroyer but for sending a bunch of people into mortal danger on a whim and even hampering their efforts to get out alive :/ way to go..
Another episode was when bob outnumbered us in core and I, being pretty tired, warped around a bit in my tempest for I don't remember what reason, and I accidentally instawarped to the 75 gate (bob were in 75) and jumped. The gate was clear I think, but local was full of green lights and I realize my mistake and decide to get back into yz. So I warp to a safespot between the gates and use my insta. Only problem was, I used the wrong insta. I warped to the hostile fleet, in jump range of the gate they were hugging. Oh no, I thought, I have to get out of here! So I jumped. It appeared they were about to leave through that very gate, so they had a fair portion of their fleet waiting for me on the other side as well. Bye-bye tempest :/

Maybe not embaressing but hilarious in its own way..
I was in my AC Tempest and warped to engage Armageddon+Enyo we were at war with. Landed close enough to the arma to get*****y with my light tank-a-gank setup and started swapping salvos. Repper goes armor goes, his armor goes and in warps the enyo. No prob ill just dual heavy nos him so he cant hurt or scramble me atleast......wtf c'mon mods wake up!! ....pop goes Tempest pop goes pod.
Turns out the batteries in my mouse had died so i couldnt swap targets and keep the Enyo from tipping the fight to their favour 
PS. No i didnt swap batteries, i swapped mouse with my gf and have never used wireless since that day

well my most embarrassing experience was flying 10k zyd threw p3en in a ibis and getting smart-bombed at the gate. not so much the loosing of the zyd or flying a ibis but just the thought of people thinking I mine for a living.

I borrowed my bro's Apoc and jumped into a belt with a buddy of mine in his apoc. Needless to say it gets out of hand and my apoc is in structure and my reps and hardeners are off. I tell my m8 that I am in trouble and that he needs to get them off of me. Needless to say my ship pops and my buddy is like "why didn't u warp out" I was like "I couldn't I was warp scrambled," he replied to me that they were cruisers and that cruisers don't warp scramble and that I could've left anytime I wanted to.

yeah i hear that, its like always wear clean underwear in RL, in eve its never fly around with ore or minerals cos if you do get killed, you dont want minerals too apear on the killmail!!

for me its probably falling asleep in a belt in outer ring and waking up to a pod.
the only other time I think could rank close was when I attacked a rifter n outer ring belong to unknown shor/rome don't remember which one of them and it tirned out to be a wold i was in a tarnis, now I was'nt doing half bad but when I realised it was a wolf I just freaked and decided to try fly out of web range and bye bye tarnis, I got that wold a couple of days later in my raven though so not all bad.

This wasn't my mistake but I was there to witness it. No names mentioned.
I was in a mixed gang of about 30 in TPAR while Shinra were in control of Period Basis, they were outside the station and we were at max speed, aligned to the station in a safespot. The cov ops pilot took a while getting into position and the tension in the gang was mounting.
The plan was to warp in right on top of them and engage at very close range. After about 5 minutes of waiting the cov ops pilot managed to de-cloak himself on a drone and warped out, so our commander said something like..
"Screw it, I'm going to warp the gang to the station at 15k. Is everyone ready?"
"Yes!" We all said
"Warping in 3, 2, 1"
Then he just vanished from the screen... Instead of warping the gang he accidentally warped himself right into the middle of them. While in warp he said on TS "S**t, break off the attack"
Needless to say he was toast in seconds. With hindsight we should of just warped after him but I was crying with laughter and could barely see the screen .

Sadly I know who he is talking about and thats not the first time hes done something like that :p Back in our FA days the same person was fleet commander at the time and we were fighting I think Atuk at the time. Aligned to warp to a covert, FC says "Warp to MN5 gate" being a subcommander I quickly shouted over TS, NO, warp to so and so, NOT the gate... needless to say, the aforementioned FC warped right into the enemy fleet while the rest of our fleet arrived at the covert as intended :p He didn't last long, but he was avenged

My two are: I was flying from DSS to 4gq, which was a path infested with all kinds of the lowest creatures , there were some others, too... whoring the forums. How dumb is that. That thorax go 5, maybe six minutes.
The other is when i threatened to pod my cat if he didint move faster. he was not impressed, and niether was my sister.

Well during the begging of the gnw bob had 60k of zydrin in bkg station well they come up in a strike fleet. and take the station camp it get a few kill and leave. well our intel showed that they had 2 haulers with them, i figure ok well my crow can kill that and the can b4 they get me. well thx to the pas wonderfull inteilgence it was a scorpion fit for jamming. well i lock a near by claw and take him to structer before i pop.

Hmmm, FU days. The later days of FU to be exact. Sharks are doing their thing and stuff donÆt look to good. A whole bunch of n00bÆs decided to go savage and help our alliance. We quickly arm ourselves in frigs and cruisers and elects, during much laughter and screaming, a "fleet commander" from one of the less n00bish guys in the team. We set of from nonni and start to hunt mercenaries. Things donÆt go well; we manage to kill on cruiser in 2 hours. Starting to get irritated a corp. mate of mine tries out the flagging system by shooting at an alliance mate. He gets blown up and in the confusion reports the other player as an alt spy or something equally funny. More ppl open fire and whops the fleet is decimated by concord.
Swearing constantly I explain the situation to the others and get the fleet back up running. We find a cruiser and kill it off. Everyone is happy. Then from fleet command comes a message to get our asses up to c4c and protect it from some thing or another, Think it was BoB but IÆm not sure. I volunteered as scout and started to warp out in front of the fleet. In torrinos I notice a bunch of hostiles in local. Sitting at the gate I suddenly see a bunch of hostiles warping in. I notice on TS that my friends are just on the other side of the gate. Cursing loudly I tell them to jump in and engage. I decloak and engage too. As I see my mates coming into system I get the notification that concord is scrambling me, crap! I scream on TS to call of the attack but to late. pll open up and dies, confusion arises as to who did what and communications breaks down for a while. ppl get into their spare ships and launch again, only to meet Mr. sentry gun and die again. In the end I got booted from the gang and they proceeded to go get ganked by a 30 BS fleet.
The embarrassing part is that I cursed the fools shooting each other just a few minuets before I managed to get a fleet to kill it self.

Webbing and scrambling the station in H-PA when trying to kill some noob shuttle. I only lost a vigil, but oh the shame! And mind you, a station doesn't have a small halo effect when webbed, EVERYONE saw this. Even the people in 9-2 I think.

fleet patrolling around paragon soul late at night.

cant leave so nothing to hand except an empty pint glass. wot would any other typical bloke do? you guessed it.
but, an hour or so later, more beer down. in a drunken state I pick up my own pee and take a big gulp of it

Seriously, thats just wrong You need to learn the fine art of toilet runs when on a fleet op ; Jump in a system, align for destination and run for the toilet and back quickly before you get left behin after the commander gangwarped you =o

we are in keberz... one corp member decides to jump into hedgp, OMG he says, hedgp is clear except for 1 -10 target, he is sitting 15k from gate, my corp mate stays cloaked and asks for immediate help..
Another corp mate hears his cries for help and warps to instant range, tells the other guy to engage and he will JI and help... he engages and my corp mate jumps in....
KABOOM, my corp mate gets owned by the sentries WTF.... my other corp mate yells on TS, OMG their is 70 ppl here and 4 medium bubbles, KABOOM.
My corp mate had went to lansez thinking it was hedgp.. my other corp mate jumped into hedgp thinking he was there.
Closest iv ever came to wetting myself thru laughter.

Well, I just joined a new corp and they tell me to move all my **** over to HQ to begin some major operations. Dandy.
So I grab my iteron V and load up most of my junk and set destination to the HQ. Quite a distance away, though, especially after the Cold War patch, so I decide to go watch TV for a while as my ship is afk jumping. It was quite a while after when I suddenly hear an explosion. I turn around to see pirates have locked onto my indy and immediately jump to a moon. I'm screaming at my ship to align as my shield is penetrated and armor is going down. And I made it! I managed to escape with only a bit of hull left intact.
Congratulating myself on a narrow escape, I plan on another jump to a moon for good measure before logging, then returning later when the coast is cleared. My indy comes out of warp, and I immediately set it to warp to another moon and the ship zooms away. As it's coming out of warp, though, I don't see a moon anywhere. Wondering where the hell I am, I check the top left and it reads that I'm at the very stargate I escaped from. I DID NOT JUMP THERE!! WHY WOULD I DO THAT?!?!
Only then did I notice that my autopilot was still on, and my iteron V met its sad fate with most of my equipment in it.

Loosing my apoc to npcs.
Just logged in for another night of npc grinding to get some isk and a nice sec, start engaging a spawn and decide to forum ***** a bit. After a little while I notice alot of red on my hud, to my horror I saw I was down to 16% structure becuase I hadn't turned my tank on. Anywho, finally get out of the belt with 4% hull left, can't be bothered going back to the POS to refit so I just repair my armour (the spawns weren't that tough anyway).
Npc for a but more and log out still with a crippled hull.
Come back an hour or so later to do some more. Go npcing, get bored start forum whoring and suddenly see a flash on my screen. I forgot to turn on my tank again and npcs got my apoc

My most embarrasing moment had to be when I had come back from a break and was in a fleet with EXO. We were sitting on a gate in Torrinos and I had just hooked myself up with teamspeak. They were warning people not to fire on anyone because we were not in 0.0 and I pressed my button to chime in about it except I had bound my push to talk to F1 and promptly shot the gate in my Armageddon.

Getting annoyed with how slow we were moving so i go semi-afk in low sec space going towards empire, look back just in time to see my tempest get screwed at a gate
Another would be me falling asleep in a belt mining at 4am and having my brother call me to wake me up
Lastly would be when i was PURE empire noob, just got my megathron to mine more with and had a civilian shield booster on. Learned the hard way modules automatically turned off back then after like 2 hours so in the midst of one of my all night mining escapades i come back to see my megathron into structure from some 5k blaster NPC's....

Having my ship AND pod destroyed by splash damage from a single torp my gang mate launched at an intruder in our space. Long time ago... Usually takes em 2 torps these days
