I don't know if I've ever seen macros in action. I heard some chatter in Lustrevik and even had some ore stolen right in front of me because the person thought I was one.

Macro miners usually have unpronoucable names and lots of numbers in it. Most have the names very simalar as the script creates the account with simalar names that are random and gibberish.
Next up. They are like clockwork. Hauler never misses a jump in or hesitates. And it doesnt matter how long you sit there an talk to them they will never respond (because your talking to a program). Also another sign is all the ships in the mining group is all bunched together in a ball.

To see a professional team in action pop up to Elgoi and watch the mings in action. They are uncommon as they are a large team and 18 accounts cost a lot of money.
The majority of macro miners are a small team of miners, often with a consecutive number at the end of their name. This makes it easier for the macro program.
Remember there are also different levels of macro's. Those that run the complete operation allowing the person to be asleep / at school etc to one that simply drags and drops the ore from the cargo hold to jet can and no more.
The whole operation macro's are very procised in their actions as everything is done by program. People often will empty the cans to see if you are macro-ing, no responce to an ore theif is a bit of a give away.
The lack of response on local however is not that good an indicator. With multi-ple accounts and corp chat chanels many people like myself never look at local, unless I am hunting that is

I've had my suspicions about a group...
A bunch of three characters all barely 2 mnths old (within 2 days of each other) same corp history and in barges (Covetor). Similar names too....
The usual "like clockwork" behaviour, and especially when the hauler picks up the ore, it may do the "I'm stuck wiggle" for quite some time. A piloted hauler would sort this out straight away.

if you have buy orders set up for trit you often get people with names like kjfddfshdk1 and wiufefoiw2 selling trit to you. I use a locator agent to find these people then go and knock them about.

You can sit there and petition them, bump them, steal thier ore, blow up thier ships, but yet they just keep comming and comming.
2 weeks I busted my rump to rid Osaumuni of the so called macro miner, and when they finally left it was nice and quiet for 2 weeks. Then all of a sudden I log on and theres 30 people in local when there is at most 10-12 on any given day. Now there are 3 more groups in the system.

easy way to tell. u and a couple of mates make 3 alts with names like "BargeGanker", put them in kestrel, fly up and orbit the barges. see what they do
if they don't react, blast the bastards. then wait for the concord timer to run out, while watching them warp back and forth from the field on their macro (macro doesn't stop on ship death  ). if u feel pity, go back and PK them to put em out of their macromisery
and make sure you have an alt with a name like ZOMG LOOTZ to collect the strip/ice miners they drop
if u wanna find some good macro targets, go to malkalen. i petitioned em all, but afaik, theyre still there

I tried educating myself on this very thing last night in Lustrevik. A few months ago, you would see some regular miners there - local builders who needed trit, new players just starting out, and a few afk indy miners. Recently, there have been many threads about this scourge of macro mining, so I guess it must be getting worse. Certainly, one of my friends who spends a lot of time in Lustrevik has been complaining more and more often about the system being packed with obvious maco miners. I honestly thought people were exaggerating.
I was quite shocked at what I found. Belts completely wiped out, or maybe just a few tiny veld roids left. I came across my first group of miners and approached them in my Tempest just to see whether they would say hello or anything like that. They remained oblivious, bunched up in a little ball in mining barges mining away happily with a hauler waiting nearby. Nothing particularly suspicious about them really, but I thought I'd watch for a while anyway. After about 5 minutes, they all started moving and warped out of the belt. I noticed that the can they had been using had been renamed to an obscene insult. Friendly guys, obviously.
Next I came across a group of people all called Tommylee with different numbers on the end of their names. They seemed a lot more obvious. A friend had already petitioned them, then demonstrated the macro in action by blocking the inbound hauler. It just kept trying to go in a straight line directly to the can even though there were two battleships in the way. Whenever the mining ships were bounced out of range, they would return to where they were and sit doing nothing for a while, then start mining again. Naturally, nothing was said, and there was no response to questions in local. Eventually, whoever was running all the accounts must have come back to find that nothing had been mined, and all the ships in the belt warped away.
I have to say that I really despise people cheating the game like this. I couldn't care less what people do with ISK acquired legitimately through normal gameplay, but blatant exploitation on this kind of scale will kill the game if it's not stamped out.
The other bad thing is the stupid names are a massive immersion killer. They are pollution. They are scum. They should be banned from having electricity! OK, going a bit far there, but I like this game and don't want it ruined.

So you think you've spotted a macro op and want to prove it?
1. Scoop all the ore out of their can into your can
2. Target your own can and blow it up right in front of them
If they continue to mine and jetison cans without reacting to your blattant act of destruction you have an automated mining op. Move on to step 3:
3. Blow up as many as you can before Concord pwns you.

If you steal ore from a macro miner you are benefiting from their cheating and are on a sticky wicket - unless you petition them first. CCP don't seem to mind that since they get more petitions about macro miners.
If you think about it, a macro mining operation could be a lucrative thing to have in your system if you go and steal their ore without petitioning them. So, you are in line for a ban if you steal their ore without the petition first.
