In my day...
...people used to suggest emphasis within a sentence by using italics. Am I missing something as to why every man and his dog now expresses it with _underscores_ on the internet? It's not like there's not an italic option with practically every message board system etc.

I use asterisks. 
A long time ago, in a galaxy far, far away, you couldn't use italics on the Internet(tm). Message boards didn't have it, IRC clients didn't have it, et cetera. Thus the spread of _word_, *word*, -word-, WORD, ~word~, et cetera.

I do THIS when I want to emphasize something.
I'm also too lazy to hit the italicize button in the Reply screen.

Yeah. If I could italicize on the forums by using keyboard commands it'd be fine, but having to use the mouse, select the text, then click on the button to change it to italics is too much work sometimes. -Wrayeth

Umh, you can. just write it yourself. I NEVER use the formatting buttons, and I still link, bold and italicicitize. Take longer to write the word than to write the formatting.
It has been a few years since I last saw _THIS_ being used. It is a remnant from days where text formatting in text based communication was limited. You find them being used on tightly knit game modification forums. Eve might be hardcore among MMOs, but Eve is still based on generic gamers (the rate of counter-strike players on these forums seriously makes me sadder than dead pandas). This is why you won't see such nostalgia here.

you can just manually type in the tags you know  i always do that for pretty much every tag i use, cept links and some emotes

i prefer *this* as its compatible with most things and is easy to read and easy to recognise
imo superior to italics as its picked out faster :)

Try using _these_ in Word and *see* what happens. 
Underscores are used for italics, and asterisks for bold emphasis. I've no idea if this is something that originates from typesetting before the days of word processors etc. but it seems likely. It's still useful when you send email in plain text for example.
