I usually make them at about 11 or 12 km, but does anyone know exactly how far past the point of interest you have to fly to get an exact insta bookmark?

If it's for a gate or a station, there's a wide margin of error ... those things are BIG.
To get an exact insta on eg. a cargo can, I'd suggest 10km past it. But I've never tried to insta-mark anything that small, so not sure. ________________________________________________

You can turn on the "size" column in the overview to see the size of the object you're making the insta for. You want to be 15km from the center of the object, so just subtract half the size of the object from 15km to get the distance.
For instance, something 10km across: you want to be (15 - 10/2) = 10km away. The size of gates varies quite a lot - Minmatar gates are huge, but those of other races tend to be smaller.

For Caldari, Gallente and Minmatar gates I get the optimal results (warp directly into the gate structure) when I set the BM when my range counter shows 2700m past the gate (If I do it at 12000 as most people suggest, I land WAY behind the gate when warping to the BM). For the smaller Amarr gates, the magic number for me seems to be 1300ish, though that does somehow not work 100%. The Amarr gates are a biatch to bookmark... Your best shot at finding out what works for you is *gasp* trial and error.

2700m and 1300m? That's 2.7k and 1.3k. Seems a little.. off.. to me.
Trial and error is great, but what works for other people will work for me and vice versa. I think it's better to share the wealth of information out there instead of repeating history.

So for 15 km warp-in these values must be substracted -5km. But I think that this matters only for Amarr gates, because they are so small. I go for 14-15km and it works for both 15km and 20km warp-to.

Since you can activate a gate from 2.5km, so even the 2.7 one is fine. Thats only like 2 or 3 seconds for a BS to make up the extra.
From personal experiance, I believe the (random) dropout point is within 1.5km from the actual point.
P.S. "Warp-to" was nerfed because of all the people in empire bumping eachother 300km from the gates... So its almost impossible to use a BM to land on a can anymore. You'll regularly end up 2 to 3 km from a can, but they still work perfectly fine for Gates and Stations if you know how to make them properly.

Gates: 12 or 13 km, regardless of gate, and you'll land within 1km on a 15km warping.
Station: Try to sit yourself in the middle of the station. On most stations you'll land in docking distance regardless of where you warp from.

i can't think of a station where this doesn't apply... unµess you warp frmo directly below or above on some of the stations

the kinda tall caldari one with the bend in the middle doesn't work well with this approach

I find that 9km past a gate works fine for me - it usually drops you out of warp just before you go past the gate, so you can still see it in space to right click on (if that's the "jump" method you use).
Slightly off topic - show stargates in your overview, and you can select them while warping, and left click to jump.
