I have just tried the defender missile in combat for the first time, and I can't see that they are doing anything against incoming missiles.
Can someone give me the rookies guide on how to use them correctly?

OK then, let me rephrase the question. Someone on rookie help suggested that defender missiles don't stop or destroy incoming missiles, but reduce the damage they cause.
What is the most effective type to load in a second missile launcher? Defender missiles to protect yourself, or more offensive missiles to kill the enemy quicker?

Defender missiles do damage to incoming missiles if they hit. Sometimes it takes a few Defenders to destroy some of the larger missiles.
To be honest, you're better off loading regular missiles and killing your opponents faster, especially at lower "levels" of the game.
To use a defender, wait till you see or hear an incoming missile, then trigger your defenders. -- Lil Miner |

Unfortunately, there isn't any way to get Defender missiles to fire automatically at incoming missiles. Thus, the only way to defend yourself is to see the missiles coming and activate the launcher manually. At close range, this is basically impossible, though at long range you have enough time to react.
If you want to use Defenders, put them in a rocket launcher. This gives them the rate of fire necessary to have any effect. When you activate the launcher, it looks for missiles. If there aren't any around, it deactivates. If there are missiles in the area, it will fire Defenders until it can't see any more missiles, and then deactivate. When you get fired on again, you will have to reactivate it yourself.
It would be great if CCP would allow a launcher with Defender missiles to be put on standby, and fire automatically at any incoming missiles. Currently, NPC ships without missiles pose little to no threat compared to missile-equipped ships.

Defenders need time to lock, then fly to and hit enemy missiles. I find they're pretty much useless at less than 20km. If you just want to pack them to stop enemy missiles, then I'd advise sticking them in a rocket launcher, which has a faster ROF than a missile launcher.

That is what I suspected from my experiments (which fortunately I survived, but only just). As you say, without this ability the sheer practicalities of shooting down incoming missiles one at a time are just too great. Better to kill the bad guy quicker.
Thanks all, really appreciate the help. 4 weeks on Eve and oh so much to learn.

would a smartbomb do the job as well? Or basically, does a, say cruiser sized, SB enough damage to destroy incoming rockets?

You can use a smartbomb to destroy missiles, but you need to time it correctly, which is tricky as you need to time the smartbomb delay to hit the missile when it's in range.
I've not heard of defenders or smart bombs being able to destroy rockets though. I suppose it's possible with smartbombs - rockets are too close range for defenders.
