Yes, my main level3 agent has referred me to 2 other level3 agents.

I have 2 lv3 agents that i use. Both are Caldari Navy. between the 2 i have done 160 missions and been refered to just about every lv3 agent availible in the Caldari Navy. One of them is excellent the other is Awesome.

No.
Getting a lvl4 was possible some time ago due to an bug, but not anymore. THe lvl4 don't give you anything ATM either way.

My Level 3 gave me another level 3 after 103 Missions, another 30 missions and i got another

Got 2 additional lvl3 from my lvl3 agent between mission 40 and 50.

I have gotten 2 additional (bad) lvl 3 agents after 40 missions.

Has anyone received a referral from their L3 when they were at their corp or race faction cap? (+6.0 corp or race faction)

Yes. Have had 6.0 since mission 10, have recieved 3 implants and a referal to another level 3 at mission 47. No failures, no changes in my 6.0 rating.
