Agents seem to be failing missions which the player has not accepted. If you look at a mission and close the agent screen, although in the mission log it shows as offered (not accepted) you get failed on it when it expires.

I noticed the same problem here but I don't know if it's a new bug or just something old I didn't recognized before. Anyway, it's an annoying thing.

its the good old b ug many patchs ago... it was the reason i quit doing agent missions...
and now its BACK!! when ppl are doing the most agent runs 
support Idea: QuickInfo an alternative to ShowInfo

Workaround for this problem is to bring up agent window but don't 'ask for work'. Check the offered mission in your journal, if ya don't like it just close the agent window and come back later

we are talking just that... you dont do anything but if you just talk to the agent without even asking for work you get the missions... it auto offers it so hmm the best way is dont talk to your agent if you dont want any missions 
i mean dont open or anything with him at all...

same problem here... it only happens if you finish a mission where you have to bring back something to the station where your agents office is.. you will get an active mission even if you didnt accept one.
If you finish a missions (couriermissions) in another base he will just quit the talk with "my office is located ..." and no new mission is active..
hope this will get fixed soon
