How do they work? For example, i downloaded the client version last night. Does it automatically update?

Before you can log into your character, the game client will have downloaded everything you need, including all patches and updates. We all play with the exact same game client (exception being h4xx0rs).

Patches that add new content as well as patches that fix the patches that add new content are required to play EVE. I believe they keep the client download updated with patches so you should have everything you need. If you can log into the game right now, then you have the latest update. After they add a patch, and you try to log in, a message will appear saying that you need to download the newest patch and giving you the option to open the link to download it and its pretty much self explanitory from there.

Everyone plays in the same game world so everyone has to have the same client version, if you can log in then you are up-to-date. If a new patch is released then the client will try to auto-update, however when you have 100k people auto-patching at the same time it can get a bit slow so there are usually download mirrors available so you can manually patch.
Also, all of the updates/patches are free.

Yes, the client should automatically update when new patches and expansions come out.

Avatar updates are weird - it can take a week or two, sometimes longer. It's an automated system of some kind.

What good are actions if there's no one to tell the tale afterward?...

Depends if its currently working or not  . If its not working, months are possible, if it is, a week or so. For some reason it tends to break, often, for long stretches of time

No, the server admins might pull out their magic scripts and update the forum portraits someday. You'll just have to wait.
There is nothing you have to (can) do to make this happen sooner.
