Lets talk about slip-ups.
I JUST did this one, Warped into the "worlds collide" blood raider base in a stealth bomber...
Anyone want to beat it?

Dominix, all faction armor hardeners, all T2 drones. Warp into Mordu's Legion forces, launch drones, go afk.
Come back to pod. Suddenly remember that the last mission I did was against Sansha's, and I had my faction *EM* armor hardeners fitted...

I met benco back in 03, worst mistake of my life
Along the way i lost a few bs and dreads here and there, for plain forgeting to turn on the reps on.

lost count how many times ive done this one... agent gives me mission, I go to mission area and kill all, think to myself "I pwn, look how fast I killed them all! RAWR!" get back to agent and she says something like "nice work, now go back and blow up the armory like I asked you to"

Doing a courier mission into low sec, sneaking past 2 gate camps, sweating bullets and breathing a huge sigh of relief when I finally dock...
... only to find I've left the package back in the agent's station.

A few months ago I decided to go rat in 0.0, and it wasn't such a bad experience, made quite a bit of isk, and got some nice mods before getting podded by the owners of the "land" :).
I acquired a Domination Gyrostabilizer from a named NPC spawn worth well over a million isk. At the time I dismissed it as a lame named t1 item. I didn't know of these things called "faction items".
Fast forward a few months, until yesterday. I was moving my HQ to a different system, and while I was packing this Domination Gyro caught my eye. I decided to check it's value. I nearly creamed myself when I realized this one item that has been gathering dust in my personal space for months is worth almost double what I have in my wallet (yes..yes poor n0ob syndome).
I quickly pack this baby into a shuttle, set destination to Rens, start the AP, and minimize the game (to read pointless crap on the forums). A few minutes later I maximize the game, only to see my pod floating near a gate....
Well it turns out that my understanding of negative security status and its correlation with venturing into empire space is flawed  . Was pwnt by Ammatar General, and of course the module was destroyed as well...
I won't be getting over this for quite a while...

was in my brand new ferox just after exodus, warp scrambled a noob in a 0.3 system to teach them a lesson, let them go then headed for dock.
station sentries take me to armour while i franticly click the nearest warp point
right into the gate and gate guns put me in a pod

Just played for two months first time I joined. Recently got back and sifted through my hangars, trying to get to grips with the game. Sold crap. One item sold for 16600% over market. Heh, I even ssed that noob. Half a mil for a domination standard missile launcher. Not uber but combined with laughing at him I feel a *bit* stupid.
When I got into my ferox I promptly warped out from a mission to a pos.. I have a tendency to see warp as final and go do other things in the meanwhile. Pod. Same thing but warping in to missions where I forgot to turn on tank or that annoying lvl 3 where first warp leaves you at an acceleration gate but the second time it warps you right into it.
And finally today autopiloting from shuria to nahyeen I forgot to take the 1 jump longer safe route. Take the dog for a walk and my condor + pod was killed by a sniper pest. 90 mil in implants.
Well, they were cramping my style anyway.

After the ECM Burst module was balanced (it used to be strong enough to jam a mining barge, used very little cap, and CONCORD didn't attack you for using it on someone), I decided to activate it outside a station. (I didn't know about the changes yet)
I was within 3000 meters of the station, a CONCORD battleship, and somone that was locked onto me. My uninsured blackbird lasted about 4 seconds under fire. From then on I made sure to insure ships before I did anything stupid. Strangely enough, I did not get a security status hit for this.
A few months later in the Caldari COSMOS, I lost a Caracal. It was fitted with the usual missile launchers and a smartbomb (Always had one for the rats that orbit you really close, yes I know I was asking for troble), and I was in the Guristas Spy station along with several other people.
I activated my smartbomb when 4 or 5 rats were within range of my ship, but unfortunately someone's Hobogoblin was in range too .
My eyes widened in horror as Aura said "Your security status has been lowered" and a large CONCORD fleet formed 30 km away. After I docked with a station in my pod, I noticed I had lost 0.5% sec status for attacking the drone, and a further -2.0% for assisting in a ship kill 5 minutes later when the Hobogoblin was destroyed. (drone kills count as ship kills?  )
The caracal was insured though

In my early days i got concorded into pod numberous of times.
Mostly while in fleet battle or during corp practice, because i had the tendency not to focus on what to target, lagg stupidity, you name it.
So while shooting, i found out that i was shooting a can, instead of a corp mate. blam end of ship.
Or use SB in high sec, which of course did damage to it's surroundings.
Lost quiet a couple of ships, before i got that under control
It still is risky during fleet ops, where the lag is tremendes, guess that will never go away.

Back a year ago in stain, forgot to turn on my hardners after a CTD while ratting, cost me an apoc
We all do stupid mistakes, its only bad if we do them again

I did the worst thing i didnt have much money so borrowed 20 million or so from a corp mate to buy a coveter. I did a favour for someone about a month ago to rid his bounty and got a -2.2 sec rating. So here i am in my coveter going to a system 8 or 9 jumps and set my autopilot. I chose the safest route all green systems but totaly forgot about my sector rating and went into a 1.0 afk . needless to say come back to find myself in a pod 26 million isk down the drain and a 20million loan to pay back. Must remeber sector ratings :)
