If you click on the "Terms and Conditions" link on the page where you purchase the Collector's Edition, it says this:

With that said, I purchased it yesterday and beside a confirmation email, I am yet to receive any game time or the other digital items to my account.

I may have bought the collector's edition through the store as well, not sure. I went through the account page at first but I may have ended up in the store somehow. It did ask me to login with my account at some point and I did. But so far I have not received any game time added to my account.
CCP must address this issue. It is unacceptable that whether you receive the 90 days game time or not depends on a particular link you click. Both options sell you the same product from the same company during the same promotion. It doesn't say anywhere in the promotion "DON'T buy it through our store or you won't receive the additional game time". I wouldn't buy the collector's edition otherwise (with the shipping costs it makes it totally not worth it without the game time).
I opened a support ticket regarding this issue. So far I haven't heard anything back.
Will post if there are any updates.

What about people who bought the collector's edition via the EVE store during the promotion? It did ask me for login but I didn't get any game time added to my account. How can I resolve this?

According to CCP, you should have had your account credited with the 90 days game time as soon as you submitted the order and payment was cleared. But if you bought it through the EVE store, apparently you are not entitled to the 90 days.
Got a response to my support ticket - CCP thinks it makes perfect sense that even though there is absolutely no information or any warning that you are not supposed to use the EVE store for this, you are not going to get your 90 days. Doesn't matter it is the same product, price, store, company and time period. It is my fault somehow.
And they have not responded to my order cancelation request and shipped it anyway, hours after I asked to cancel it.
Yes, I am upset. And I do feel cheated out of my 90 days game time.

It may be not a "False Advertising" per se, but it surely is misleading and confusing. As these things go, I don't think this was done on purpose but rather due to typical incompetence.
I guess that someone at CCP came up with this promotion idea so they quickly wired it up to the account management. When they realized that people can still buy it through the EVE store and miss the promotion, they figured out it's easier to just claim that this is "by design" and the two are totally different things than to go and clean up the mess. including retroactively applying the 90 days game to people who already bought it.
This is rather ridiculous. The promotion doesn't have any clear information how to buy the CE to have the 90 days applied, there is no warning that you should stick to the process of buying it from there or else you'd miss the promotion and the EVE store DOES ask you for login as well. This is as confusing as it gets. No typical user would even suspect these are different products and their purchase will result in different outcome.
The right thing to do would be for CCP to take responsibility for this mess and clean it.
But as someone already mentioned in this thread - this is EVE, you can't be suspecting enough...

Don't know if you can do anything else besides opening a ticket and trying to draw the attention of CPP to the issue on the forums.
I opened a ticket about this 6 days ago and it is still not resolved. So I am still waiting.
Musterband, the company that distributed these for CCP ignored my cancelation email request and shipped the package anyway. Now they say that in order to get refund I can refuse the DHL package when it gets to my door. Not sure who will be then charged for the returned shipment so I asked Musterband about it via email. Still no response.
