Here is my current setup: High 5x 150 mm Railgun II
Med: MWD, Web, 20km Warp Scram
Low: 1 med armor rep, x3 N-type Hardners, 1600mm plate x5 med drones
Biggest question is I feel really vulnerable to a nos, so should I drop a rail for a med nos? Or any other changes.

You're going to get a lot of "Don't use rails on a 'rax" responses. But I'm going to answer your question. But really, don't use rails on a rax. Especially small rails. If you look at the fitting requirements, Heavy Ions give you the best bang for your buck, giving you a good bit of damage and also allowing you to create a decent tank.
Make sure that the ranges of all your weapons match. With no additional skills, the range of a Medium Nos is 11km. With very little skills, you can hit with AM S at about 15-18km. You seem to have some skills, simply because you can use T2 small rails. So, even if you had a Nos, it wouldn't benefit you at all.
If you are going to stay at or under 10km to use that Nos, you might as well train for T2 Blasters and just use Null. With Optimal and then Falloff, you should be pretty close to that.

Small rails on a Thorax are just kind of pointless. If you're going to rely on your drones for damage (and you will, since 5x small rails = pathetic dps), why not just use a Vexor? If you're going to fly a Thorax, fit medium guns. Electrons/MWD/800mm plate/medium rep will fit with even average skills, for example.

Small neutron IIs end up giving you about the same DPS as medium electrons after considering tracking and all that. And you get to fit a bigger tank.

1. Don't use rails on a Thorax, unless you just want to fly around ganking frigs.
2. Don't use small guns on a Thorax, that's just so 2005 (and back then, they only worked because the people using med weapons didn't fit plates). If you want to pew pew with small guns, Nos and kill with drones, fly a Vexor instead.
Suggestion: fit Heavy Electron IIs, and use a Small Armor rep instead of the Medium one. The plate will keep you alive very, very long with just a SAR II against any other cruiser.

Light Neutron II + Void/Null + 1600mm > Heavy Electron I + AM + 800mm ----------

It all depends on what you are doing whit the rax. If your are running missions keep our t2 150 for picking of cruisers and drones for frigs. If you want to do some pvp go for the 4xHeavy Electron IIs + nos + t2 Small Armor rep to keep the 1600plate tank running. Works for me!

I think the question you need to answer is "how good is my tracking with medium rails at 19km vs. cruiser-sized targets that are approx. my speed or slower?"
I know rails are better than artillery at tracking, which is odd considering minmatar have no range bonuses since the tempest changes. However, I have never trained to use them, so I really really can't answer that question.
If the latter worked, I'm sure there would be plenty of cap-boosted nanothrax users out there. Rails are much more economical on cap compared to blasters.

Err... I'd like to see that fitting. After fitting 4x Heavy Electron II's and a Medium Dimi Nos, I have less than 500 PG left. And 1600mm RT + MWD equals about 665 PG...
