Does it vary by sector security rating?
Do the CONCORD ships actually warp in from an actual place in the world or are they spawned on a timer? In other words do the responders actually move from one area to another or are they simply created to meet the need?
Do they arrive 15km out from the "scene" or right on top of it?
Can player pirates outrun/outfight them or are they impossibly fast & tough?

There are ways of evading CONCORD, but escaping CONCORD can lead to a ban.

I think they just spawn as needed, although I am not sure on this.
Not sure how far they appear from the scene, as it doesn't really matter. They show up and WTFBBQ the offending ship, quick as hell. The last time I witnessed it myself, I was with a new corpmate when he accidentally targeted and fired upon someone else's can in a 0.6 system. Before he could say "Oh shi...." he was in a pod. 
Escaping Concord is considered an exploit, so best to not even think about methods of achieving that. 
Edit: Oh, one more thing... if you do find yourself on the receiving end of a Concordokken, don't undock in another ship until I would say at least a full minute or two AFTER the aggression timer has gone away... sometimes those timers are a bit less accurate than you'd think.

Yes, they just spawn when needed, distance varies, but they will follow till they catch u, they have awesome jammers, and if u get scrambled u need more than warp core stabs to escape them... :)
They also spawn and keep spawning till they arent needed.. So 1 wave will come in... a bit later another wave, and another, and another, and another... until you die
Fighting back wont help, they just get replaced by more waves of them.
There was an incident in yulai a good while ago where concord and the sentries were tanked and a lot of people got podded in secure space, so concord were uber boosted.
Concord in their current form are untankable by anything ingame (other than the special GM uber ships of course) :)

It took exactly 20 seconds for them to respond, when I was attacked in 0.5 space.
No idea if it's any faster in higher-sec.
They arrive within their warp-scramble range and engage immediately.  _______

Myself and my corp are total Hi-Sec Non-Pvp. We put as much thought into survival in Hi-Sec as other do in Low-Sec.
The law of survival in Hi-Sec is with the VERY RARE 'suicide gank' is that your shields, armour and hull have to be tough enough to take a few shots until CONCORD can get there. In Hi-Sec guns don't matter, ship survial does. CONCORD have the guns...
Another rule of thumb..If they can't target you they can't fire on you.
If they can 'bait' you into firing on them, like stealing from your jet can, or jet one themseleves under the guise of "helping", lock target on you just to annoy you, then CONCORD will not do anything because YOU are seen as the agresser then if you fire on them.
The reason its called a 'suicide' is because the attacker is TOAST, no ifs, ands or buts about it. You CAN be attacked in Hi-Sec by members of a corp that has delared war on the corporation you are in without CONCORD intervention.
A bunch of us made some noise about people being able to attack you at all in Hi-Sec (eve with war dec) but it fell on deaf ears and we were called 'Whining Care Bears' , so be it.
0.5 space, as well as 0.6 space take a quite a few seconds to have CONCORD show up. But USUALLY people out to kill in Hi-Sec aren't going to use a battleship outfitted with cruise missles :)
Survival is key, let CONCORD deal with the scum.

Yeah, I do that in Jita when I pass through now....I lock on several newbies.
I had the same thing happen to me when I first started, and it scared the crap out of me, heh heh. So I do it to other newbies now just for entertainment. Locking wont hurt em.

Locking noobs in jita is actually quite fun ^^. Those rookie ships and badly fitted frigates pop pretty fast when concord shows up ;D

I started back in beta and there wasn't a Concord. You could attack someone's mining drones and steal them without any recourse. I came back from beta a few months ago and was wtfbbqpwn'd by Concord for, guess what?, firing on someone's mining drone. Needless to say i was sitting in a pod quite quickly.

Yep thats true, just enough time for you to activate your scrambler and open a ransom chat with the pilot before concord comes to show you that you arent in low sec
