A time synchronisation issue.

At 14:20 the active database node (the database is a cluster of machines) reported that its clock was out of sync by 6 minutes. This is too much error to correct by slewing and therefore it stepped its clock. This correction was outside the window for the heartbeats, each node in the simulation cluster sends a heartbeat every 60 seconds with a window of 180 seconds, or 3 minutes. Therefore on the next heartbeat all the nodes in the cluster were considered to be way outside the window and promptly removed (on the assumption that they were dead and not responding).

The database is the authoritative source, the proxy nodes get their time from the database and deliver to the clients, the simulation nodes get their time from the database as well and then the simulation nodes and the clients run simulations in parallel (the simulation nodes run a global simulation with all entities/actors, the clients run their local simulation with the details they are privy to).

The simulation would even have had great difficulty coping with this change. Normally all the machines in the cluster should agree on the time to within -¦-+ second and weird things start to happen when the discrepancy is more than, say 5 seconds. At that time the simulation nodes and the clients start to noticeably disagree on position and movement (desync) and module cycling.
