One player in my corp has Refinery lvl5, Refinery Efficiency lvl5 and all high end processing (Arkonor, Bistot and Crokite) lvl5. He gets about 96,875% refinery yield on a Minmatar Outpost, however the extra processing skills are not taken into account.
But even with the extra processing, his refinery screen still shows unrecoverable minerals?

Base yield in a non-NPC station is 37.5% not 50%.
Even with max skills it is impossible to have a perfect refine.

Think the guide says you can't ever get a 100% refine in some low yield station.
But, mein gott, those are some nice refining skills!

You see at the picture that he gets 96,875%. This is not including the extra processing skills, which gives:
Specialization in Crokite processing and refining. Allows a skilled refiner to utilize substandard refining facilities at considerably greater efficiency. 5% reduction in Crokite refining waste per skill level.
Wouldnt does 25% all together on lvl5 push it over to 100%?

It reduces the waste, not increases the yield. So with a 96.875% yield there is still 3.125% waste. Figure out how much 25% reduction reduces from 3.125% waste. Add the waste reduction to yield.
96.875+((100-96.875)*0.25)=97.65625% yield

Yea. I'm certainly missing something.
The yield formula in your case and as per Halada's mining guide is:
0.35+0.375*(1+5*0.02)*(1+5*0.04)*(1+5*0.05)=0.96875 or 96.875
So it would seem that Outpost's do take the processing skills into account.
I'm confused.

It is very possible to get a perfect refine. The skills you outlined above are well beyond perfect.
When they refine some ore, check the report that gives the details. You will see waste material at 0. I do this all the time.
If you are refining in an outpost, there is going to be some waste, plus whatever the owner of the Outpost takes. Best refine at Outpost (37.5% equipment) is 99.38%

Outpost equipment is 35%.
