In your rules & policies it states that it is $14.95 on a month-to-month basis. I got charged $19.95 for my first month, should I expect more of the same every month or is the extra $5 a one-time charge?

That initial charge is for 'buying' the game. It is the analogue of going down to EB in the mall and picking up a copy of WoW (or any other pay-to-play game) for $30 and then having the first month 'free'. EVE is $20 to buy with 1 month free play time, after that you pay $15 a month (or less if you pay for more time at once).
And if you are expecting a response from anyone from CCP here you are mistaken. These forums are moderated by a team of volunteers, on occasion a dev will post, but for the most part the only people that will see your questions are other players.

Work hard and earn ~150 - 200 million in game currency, and you can pay that $14.95 in ISK. In other words, you play for free.

The $20 cost to purchase an account is clearly marked on the website. Like others have said its not a mistake, and its a one time fee. You get 30 days free with your $20...after the first 30 days you must purchase additional tme for $14.95 each month.
