If you want more realistic insurance, let the game take in player statistics.
Everyone starts off on a low cost - high payout system that renews every, say couple of months. You can chose seperate levels of cover to cover the different costs of ships.
Everytime you lose a ship, it pushes up your renewal price. This excludes ships blown up by corporation members, CCP and environmental damages (Deadly gases). The exclusions also apply to payouts.
However, the system also takes player details into account:
Players who spend most of their time in High sec don't won't die as much - Lower Price Players who spend most of their time in Low sec have a higher risk - Higher Price. People with bounties on their head or have had one on them are higher risk. After 3 months no bounty they are no longer high risk. Players with lots of repair/self-buff skills are less likely to die, lower risk. Players with lots of weapon skills are higher risk, higher risk. Etc.
Also throw in a No Claim Discount so after X amount of months of no payout your renewal price drops.
Not a perfect system, but it's more realistic.
