Anyone know where I can find the POS fuel consumption equation?

That's the easy part. The figures shown there for Liquid Ozone & Heavy Water consumption apply only if all the CPU & PG of the tower is in use. If x% of CPU or PG is in use, only x% of the appropriate fuel type is actually needed.
Also, starbase charters are only required in 0.4 or higher security space.

The formula I heard is this:
Fuel Consumption = ((Ship Mass / 1,000,000,000) * 500 * LY)
Where LY = Light Years distance.
So, to go 5 LY a regular Frigate will take about 3-4 Fuel. A Battleship will take about 265 or so. Cruisers are about 25-30. It's super easy to set up a spreadsheet.

I pulled mass numbers from EVEMon. You can adjust the Jump Range and Fuel costs at the top of the chart. I assumed 350 ISK/unit and Maximum range (5 LY)

Your Industrial average is missing a cell, though it still gives the correct answer.
