How do I know if I'm Warp Jammed? Is there some kind of telltale sign other than I just can't warp out?
And can I still Approach something at top regular speed if I'm Warp Jammed? Will that get me out of it?

There is no sign you are warp scrambled unfortunately (other than you can't warp and a voice tells you you are warp scrambled). The scrambling ship will have blue rings emanating from it toward you.
Your propulsion won't be slowed so you can move away from the scrambling ship and get out of range. If you are webbed, however (your ship is surrounded by shimmery blue haze), you will go very slowly. -- 2d EveMaps

You can still fly away if warp scrambled. The only sign that you're warp scrambled would be the little alert message or the blue pulsating effect coming from the scrambling ship. If you fly outside 20km (normal scram range, some have more) then you should be able to warp off.

Warp scrambling is range limited. If you can get far enough from the scrambling ship, you're free to run for it. If you can kill the scrambler, do so. Otherwise, run like hell away from it and pray for distance. (A good idea to run in the direction you need to warp in) Align to something, set full speed, MWD if you've got it and keep up the guns if you're able. (Might get lucky one way or another)
If you're scrambled and webbed.....you're (CENSORED). If it's pirates, see if they're willing to let you go for a ransom. If it's NPCs, well, I hope you paid for full insurance. _______________________________

Is Warp Scrambling a hostile act?
I noticed this blue ring effect the other day when I was getting ready to jump, but as I was able to jump away I just skipped it and forgot about it.

Jumping as in jumping through a star gate? If so, they can't stop you form doing that I believe, unless you are not close enough to gate to jump and they have you webbed slowing down your approach.
Also, doesn't the sensor booster emit similar blue ring, but rather then in a cone shape directed to you, it's a ring emitted from the ship? Could you have mistaken this for the warp jamming one?

If someone was trying to scramble you and your warp drive had lit before his scramble took effect, you got lucky. More than likely he was trying for someone else.
And scrambling, webbing, sensor damping, ECMing, etc are all hostile acts. Just about the only safe thing to do to someone is scanning them. _______________________________

warp scrambling/disrupting is a hostile act and will bring Concord in high sec. The blue rings you see off ships sometimes are sensor boosters.
