The number of female avatars, as well as general internet experience, lead me to suspect there's be a lot more sex change operations if the surgery was more affordable.

Even if sex-change surgery (and all medication that goes along with it) would be free of charge, painless and without any health risks, I still wouldn't have one.
I like being a male in real-life, thank you very much.
Yet I still PREFER to have (some) female avatars in games, for completely different reasons... and I don't even try to HIDE the fact I'm a guy playing a female avatar, if anybody asks (at worst, I might say something along the lines of "who cares", but if they insist, I'd flat out tell them the truth, no problem).
Your mistake (and plenty of people's mistake) is thinking of your avatar as "you".
I merely thing of my avatar as "that person I control".
Now, would you rather have control of a decent-looking gal, or some ugly brutish male ?
I guess it's a matter of taste.
