Ok. So I get a new Vengeance, my first AF, and I want to see how well the thing tanks. So I pull up to the agent (level 2), ask for a mission, and get Tech Secrets.
I warp there and see them doing 5 damage per missile to my shields with 91% EXP res. I decide to have some fun with them and see how long I can tank. Bad idea.
The second wave comes, still nothing big (a bunch of rookies). I'm at about 65% shield or so, and it seems fine.
They finally reach my armor, and it takes a bit more damage then I thought, but it seemed ok. I start pulsing my rep and decide that once the third group comes I'll finally kill something and finish the mission.
I realize my armor is going down way too fast, so I start repping, and trying to kill them, but they're all out of range of my rockets. I hit the MWD and double click in a random direction to try to run away. First my ship doesn't listen, then it gets stuck on another ship I have webbed. Finally it runs but I'm deep in armor.
At this point I realized that the third spawn came when my shields were still at around 20%
I can't seem to get out of range of their missiles, and I'm nearing hull, so I hit the repper, turn off the MWD, and try to find where to warp.
Of course, it just has to be 180 degrees, directly behind me. I pray to god and jam the warp button. I'm watching as my brand new AF that I got no less than 20 minutes ago goes further and further into hull.
Of course, whenever you're trying to warp out of combat that you're losing miserably, it takes you forever to warp.. Or at least seems to. At any rate, I figure I'm toast, my hull is going lower and lower and I don't seem to be able to turn fast enough to get away.....
19.8 hull was left. Two more missiles and there would be no AF for me.
Mental note: Pay attention to the damn overview!

ROFL no I would have to severely punish myself if I did that. What happened was two things. My shield Explosive resist was higher than my armor (higher base with same bonus), and I had more spawns on my armor than shield. -----sig-starts-here------

In missions that don't involve you using an acceleration gate, you can indeed use MWDS. Deadspace still renders them useless though :(

I lost 4 Ravens and a 100kk Drake in L4 missions. More assets lost while carebearing in Empire, than while living in 0.0 (for the same period of time)

I lost a fully t2-fitted Nighthawk like this last week 
Was doing Damsel in Distress lvl 4 and thought I'd pull the whole spawn, i.e. keep shooting the pleasure hub structure until everything was there. Then I could bring in my 2nd char and add drones to the DPS! I'm in a Nighthawk, what could possibly go wrong?!
Oops. Forgot to insure it too, double oops. Still, only cost me about 300 mil to replace in the end, doh!  ________________________________________________________________

I have a couple stories to tell:
First one was the only time I ever came close to losing a ship to NPCs (still haven't lost one yet to NPCs, would like to keep it that way).
I was in my thorax, running level 2 missions for standing and cash. Was about a year ago I think.
Anyways, I don't remember the exact name of the mission, but it involved angel rats. I hated angel rats in my missioning thorax, as I had the thing essentially "distance tanked" with rails and an afterburner (and one repper + resists). Worked great for most missions, problem was, the damn angels could catch up to me.
Got out of that one with 120 structure left. Just as I warped out "The Fire Within" by Warmen started playing on my media player. The intro of that song was surprisingly fitting as my thorax warped away with a trail of fire and smoke several AU long.
The second time was more recent, in a war engagement with my previous alliance. First day of the war, all the carebears were excited and ready to go out and fight the good fight. We knew they were camping a gate so we sent in a scout. The idiot reported that there was just a Cyclone on the other side of the gate. No match for our carebear blob (and that is what it was sadly).
We jumped through and were ready to bring down some pain. Problem was, our scout failed to notice the two Megathrons sniping 190km off the damn gate. Our recon ship was immediately popped, and they got a couple other people that I forgot (and couldn't care less) about.
Then my myrm was primaried. The "cyclone" (was really a sleipner...) was long gone through the gate. I was moving my myrm over to the wreck of the recon pilot to get his stuff and then run away before I got tackled.
Got his stuff as my tank started breaking... ok I'm in low armor and starting to dip into structure - align for warp...
Of course the warp point was exactly behind me. Oh well, still repping... half structure... warp damnit....
Got out with 3% structure. I remember while chatting in local with the war targets they expressed interest in wondering "where the heck that myrm wreck ran off too".

edit - thank god I'm out of that alliance... mistakes like the ones made in that encounter were all too prevalent.

Best way to warp fast, I've found, is to double click directly behind you and then click warp to <whatever> right as your velocity-o-meter (forget what it's really called) drops below 25%. You will flip towards your warp target and warp faster than if you were to try and warp while impulsing in a different direction. If your warp target is directly behind you or in front of you this isn't helpful, however.
Sorry I don't have any spectacular 'close call' stories myself. But I hope that information is useful to someone

The enemy was docked and tried to pay us off but we still wanted to smack some people around before considering their offer.
I had been lagging behind in the enemy HQ system because I had been getting conflicting orders about which gate to go through but I finally got to the right gate when a person on comms started asking where I was. Even after going through the gate and me targetting him so he'd notice my name on his screen, he still continued asking where I was (no mic atm, other one broke). So I got annoyed and shot a single missile at him and got CONCORDed instantly, much to the amazement of the others.
..Yeah, I did not realize there is a difference between shooting a corpmate in a fleet and shooting other people in a fleet

had a nicely fitted harb engaged by BEES in heimatar, managed to deaggro and jump through the gate with 2hp of hull left (hate it when i get damped)
love seeing my ship listed as -1033% hp

Me and a corp mate once tried to attack a couple of gate campers that had popped another corp mate on a gate in lowsec, we couldnt get them to engage first so eventually we did, so we had the Sentry Guns on us, no big deal, everything going ok until a couple more pirates show up so we decide to warp out. I was stabbed at the time so was able to get out of the scram they had on me. But was webbed and in a Brutix so took a long time to turn and align, eventually warped out to the first thing that in my panic that i could click on, which just happened to be the Gate at the other end of the system. I got out with 5% hull only to be insta popped on the other end by the Gate's Sentry Guns.

lawl good story and congrats on the AF, yeah once u get new stuff you want to test it out and see what it can do, sometimes you get a little too daring but it's all good

Here is a trick I have used for a very long time when running missions.
Select a neighbouring star system and set destination to that system (f11 map is useful). Then bind a key to autopilot (I usually use ctrl-z and you only need to do this once).
The autopilot destination is a nice yellow stargate that is easier to find and align to, or if you are really desperate just hit ctrl-z and then try to run your mods to keep you alive till you align and warp.
It's also useful if you are being scrambled as the ship keeps trying to warp out until you kill the scrambling rat.
