I'm sure this list has gone around, and I'd like to see it updated. Anyone have some ideas on making this trinity compliant?

Well, until CCP fixes the bug where tracking & optimal rigs have a separate stacking penalty from tracking enhancers/computers, try putting 2-3 optimal range rigs on everything in addition to the tracking enhancers. Then use high-damage ammo at insane distances; antimatter @ 110km optimal, anyone?

take the plates off etc especially on the mega. Its not going to save you and just makes you slower to align out. Better off having another tracking mod in the low slot and dropping the tracking comp for a thrid sensor booster so you can lock far and fast and kill them before they kill you. Dont bother tanking a fleet ship its worthless. Melt them before they melt you. Therefore in fleet fights: Lock them first, melt them first, win.
edit: your apoc set up should be the model.
