I've been experiencing this problem for a few days, I will try to describe it as best i can.
Three times now, I have been missioning, and another player ship has warped in, presumably to loot, whatever I am ok with that. But when i see it in the overview, I cannot right click it at all, or target it- when i try to lock on, it says the ship is no longer there. Most recent time (today) it occured, it even started flashing red, but i still could not right click it to pull up info, or target it, even though it was looting.
Other times i have been surprised or ambushed, I have been able to right click to pull up info just fine, this problem doesn't occur with any degree of consistency or predictability.
Is this some weird lag effect or what? I could target every other object in the vicinity, just not this player.

I've never seen one turning red, as you described, but there have definitely been times when a ship has appeared on the OV and being unable to interact with it.
Next time you see that, double-click the bar at the top of the OV. That closes it, leaving just the bar. Then double-click again. (Takes all of 1 second) That phantom ship will be gone.
The OV can get a bit buggy at times. 
I had that happen tonight, btw. The pilot initially was at the same gate I was at, and got "stuck" in the OV when I warped off. Showed up at the next gate, even though not in local anymore. (He'd jumped out when I'd jumped in)
The double-click thing fixes most OV issues, including when the colours get all messed up. (Neutrals showing as war-targets, etc) |

I've been playing for 4 months and have seen this as well.
I think it is when another player does a show info on you as you are going in to warp.
When the other player shows on your overview and you have changed systems - have a look in local - they are not there.
I have also done a show info on a ship near a gate as I have gone into warp and they stay on the overview at the distance I was at when I went into warp.
So I think it's an overview glitch - but it's actually quite handy - you know who is checking you out.
Happened to me recently when I was coming out of a station in a mission hub - and sure enough it was a ninja salvager.

I have had overview bugs as well. Usually indeed of the variety of off grid(or even off system) player ships showing up. Tabbing twice (hide HUD, show HUD) usually solves that problem for the time being.

I've had this a few times, and I gotta say it's creeped me out before it just seemed to dissapear after a while or when I jumped system

I've had times where I will warp away and the OV will still show a ship that was there before but I've never had one appear like you describe that I couldn't target.

Happened to me, I nearly called in reinforcements as I thought it was someone trying to gank me. Turns out it was a ghost OV ship.
