The worst fits come from "oh god nothing to buy in this <hostile space> station... what the hell, buy whatever you find, throw it all in, and hope for the best".
That, or complete morons at the helm. I'd say the latter prevails though.

I once put an armor repper, shield repper, and hull repper on a Slasher. I had been in-game for a few days and an omni-tank seemed like a good idea. The shield and armor reppers were probably civilian modules too (I don't believe that small modules AND weapons would have all fit on the ship with my CPU/grid skills at the time.) There might be a killboard link to this somewhere, but I lost a lot of Slashers back in the day to Low Sec gatecamps (oh yes, I mined Low Sec ore in my Slasher on numerous occasions) and I don't know if the killers bothered posting them all. I don't post solo kill or loss mails myself so... meh.
All of my PvP opponents have been either unwitting victims who were equipped with passable PvE fittings, or PvPers with sensible equipment. No lolful killmails, sorry. But I've died many times with moronic setups, so by all means post em if you got em. To make this post relevant to this thread's original intent though, I will now post my loss info regarding my Garmon-Tanked Worm. You will notice a near-absence of mid and low slot modules.

Worst I've ever seen was my own. I took my 'cap stable' prophecy out against a group called Mercenary Coalition, long, long ago. Full mission fit, active hardners and all.
I can laugh about it now, cause I know how stupid that was. But when it happened?
So you want security? That's perfectly alright.

I used to gate camp 0.0 with a Raven that was fitted with rockets and 2x 150mm turrets. Did awesome DPS against small ships. Imagine how surprised the inty pilot is when he expects torps and you hit him with rockets. I was young and stupid.

I fit a cruiser AB on a destroyer once
actually it worked out perfect, i got the same speed as a mwd without the sig radius, and i webbed and scrammed a cruiser until the big guns could get him
but I couldn't fit jack else on it lol

Not the worst ever, but pretty bad.
Wasting a high slot on a Caracal on an auto-targeter. Shield boost amplifier but no shield booster. Cap recharger but no active modules aside from the afterburner. 4 (very expensive) arbalest heavy missile launchers on a cheap ship like the Caracal.

Dont really have a link to it but i still remember a long time ago when i was in a empire wardec corp. Had just started a war and podded one guy in his raven on a gate. A couple of minutes later he comes through that exact same gate in a untanked hauler filled with implants and whatever else he had that was valuable.
