Unless CCP introduces Fighting in Stations (with blood), use of drugs outside of the pods, and sexual content then I doubt the rating will increase. Also there is that whole "Game experience may change during online play" thing. That's just a warning that things outside of what is listed as game content may occur due to interactions with other players. For example, if a game is about bunny rabbits and is rated E that doesn't mean it should be rated M because someone started spamming shock images everywhere. So there already is a subtle warning to players that you may have to deal with things you wouldn't expect.
