What should I do?

The skill points aren't added until you have completed that skill are they?

That "feature" has been in the game since it was released 5 years ago. The counter under your portrait doesn't update until a change happens: stop and restart the skill, switch to a different skill, or if a skill finishes training. My guess is that they left it because it's such a minor priority in their eyes that it never got attention.
Just remember to upgrade your medical clone to a bigger version slightly ahead of time, so that if you die, the few hundred k SP's that aren't displayed are also saved.

Nope, they are.
They slowly increase from the start value of that skill till the limit of the next level. If training, it should always be counting upwards by the amount of SP/Hour you train for that skill.

What I mean is, the skills points you learn aren't added to the total skillpoints one by one.

The total behind the scenes is always increasing, assuming you have a skill training, but the total shown under your picture on the Character Info sheet does not update in realtime unless a skill completes, or you switch to another skill.
If you want a running total of your SP without having to mess around with starting and stopping skills, use EVEMon.

Correct
Like Letouk Mernel said, that total is not updated until you change skills.
However, those SP that you have aquired since your last skill swaps still count towards your needed medical clone. So make sure that you have a buffer beyond your reported skill points, especially when training a longer skill, otherwise you might find that you will lose skill points when podded.
