I thought mine was kind of obvious :)
Macrointel has been around a long time as an unofficial group, but was lacking any central leadership. A group of us stepped in and formed corps to give the rest some direction, with MacroIntel as more of the PR and recruiting focused of those corps.

In the US, there is a rating system for television shows much like video games and movies are rated based on their content. "Viewer Discretion Advised" appears before programs that are intended only for mature audiences. The alliance's tag, "TV-MA" is the rating that appears in an icon in the corner of the screen when the show starts.

Blancmange Interstellar Kilts
This one was fun to come up with. I thought of something that I enjoy, Monty Python in this case, and remembered a great sci-fi sketch. Created a name based on the Blancmange aliens and that they wanted to turn everyone into Scotsmen.
My recommendation is to think of something that interests you, whether it's a movie, TV show, greek mythology, astronomy, whatever... You're bound to find something cool and/or funny.

Can't really recall the details, it was quite a while ago after all. Didn't spend all that much time thinking about it though, I think I simply decided to start a solo corp and the name just sort of popped into my head.
As for the alliance... I had plenty of time to prepare for that one given the skill requirements. Came up with a short list of names I liked, along with rough drafts of possible logos etc. Later it turned out that all the names had already been used at some point.  (there really should be a way to search for previously used names... (or is there?))
Annoyed as I were I started browsing dictionaries, as they're always a good source of inspiration. The various different meanings of wayward seemed to fit the bill OK, so I went with that and here we are - 1B ISK down the crapper.

Nope, closed alliances (and corps for that matter) won't show up in the search results. I used the search function before I would even consider considering a name, but all things considered I obviously failed to take the considerable deficiencies of the search function into consideration.
